Job Overview
We are seeking a dynamic and compassionate Case Manager - Mental Health/Substance Abuse to join our dedicated behavioral health team. In this vital role, you will coordinate comprehensive care plans for individuals experiencing mental health challenges and substance use disorders. Responsibilities
- Conduct thorough patient assessments, including mental health evaluations and substance use screenings, utilizing your knowledge of physiology and diagnostic evaluation techniques.
- Develop individualized care plans that incorporate inpatient, outpatient, post-acute care, and community resources to support recovery and stability.
- Coordinate discharge planning by collaborating with hospital teams, social workers, addiction counselors, and outpatient providers to ensure seamless transitions of care.
- Manage utilization review and utilization management processes to optimize patient care while adhering to HIPAA regulations and medical records protocols.
- Facilitate intake procedures for new patients, ensuring accurate documentation of medical history, treatment needs, and service eligibility.
- Provide patient service by offering education on treatment options, medication management, and behavioral health strategies tailored to diverse populations including pediatrics and adults.
- Maintain detailed case documentation, update medical records regularly, and ensure compliance with all regulatory standards.
- Collaborate with hospital staff experienced in ICU, trauma centers (Level I & II), emergency medicine, and acute care settings to support complex cases requiring specialized intervention.
Qualifications
- Proven experience in hospital settings such as Level I Trauma Centers or Level II Trauma Centers with exposure to acute care environments.
- Strong background in hospital medicine, emergency medicine, ICU experience, or PICU (Pediatric Intensive Care Unit) is highly desirable.
- Knowledge of behavioral health practices including mental health counseling, addiction counseling, and social work principles.
- Familiarity with managed care processes, patient assessment techniques, discharge planning procedures, and medical terminology.
- Experience working within inpatient or outpatient clinics with a focus on post-acute care or outpatient services is preferred.
- Ability to navigate medical records systems efficiently while maintaining strict confidentiality under HIPAA guidelines.
- Excellent communication skills combined with a proactive approach to patient advocacy and interdisciplinary collaboration.
