PROGRAM MANAGER - RESIDENTIAL WAIVER SERVICES
Centered services and supports to people with intellectual and developmental disabilities living in their own homes in the community. PMs play a vital role in helping people live a meaningful and purposeful life in the direction of their choice. PMs are responsible for oversight of daily operations fir a caseload of Supported Living sites and for implementing procedures to ensure the quality and integrity of services provided.
DUTIES & RESPONSIBILITIES
PROGRAM OVERSIGHT
- Ensure individuals' homes and programs comply with all rules and regulations as indicated in 460 IAC 6, DDRS policies, the individual's PCISP, and all Gateway policies and procedures.
- Ensure required documentation is kept up to date and accurate for all service sites, including but not limited to: Daily Support Records/PCISP Data Tracking, MAR, monthly budgets, Meaningful Day Calendars, Risk Issue Protocols and tracking sheets, Service Agreements, and Emergency Action Plans.
- Complete and document service site visits within established guidelines.
- Ensure appropriate and timely incident follow up including completion of Section II of the Incident Report form and filing DDRS-reportable incidents.
- Ensure supervisor attendance at quarterly and annual support team meetings. Document IST meeting notes and meeting checklists.
- Ensure completion of individualized training for direct reports.
- Coordinate with the Financial Benefits Coordinator to ensure that financial needs are communicated, and benefits applications are filed.
- Provide on-call support as assigned after regular business hours and on weekends.
SERVICE DOCUMENTATION OVERSIGHT
- Generate quarterly Service Summaries and upload to the CM Portal within established guidelines.
- Ensure appropriate documentation for health and medical appointments and health supports.
- Upload Risk Issue Protocols and other requested documentation to the CM Portal within established guidelines.
- Complete monthly documentation and weekly billing remittance within established guidelines.
- Maintain individuals' Program Book and electronic file within established guidelines including maintenance of individual electronic files, monthly remittance files, and individualized training files.
HEALTH & SAFETY
- Coordinate as needed with the pharmacy and doctors to ensure medication and specialized training needs are met.
- Ensure supervisor attendance at psychiatrist appointments.
- Coordinate completion of maintenance as needed.
- Ensure environmental safety of service sites and vehicles within established policies and procedures.
COMMUNICATION
- Communicate with case managers and other support team members as needed to ensure service provision is consistent with PCISPs and provide conflict resolution as needed.
- Communicate incidents to the Director of Residential Services timely.
- Report issues requiring formal internal investigation to the Director of Residential Services.
- Demonstrate/maintain professional internal and external relationships, cooperation and partnership with all Gateway employees, managers, and the people we support.
EMPLOYEE ONBOARDING
- Work with the Advisory Board to identify hiring needs and to interview and onboard new employees as appropriate.
- As needed, conduct assigned sections of new employee orientation following established guidelines.
NEW ADMISSIONS
- Assist with attending transition meetings and participating in the admissions process including arranging moving teams, truck rental, and new home set up.
LEADERSHIP
- Provide direct oversight and supervision of subordinates.
- Complete Performance Observation Logs quarterly.
- Conduct timely and accurate evaluations and disciplinary actions following established procedures.
MINIMUM QUALIFICATIONS
- At least 1 years of direct experience working with people with intellectual and developmental disabilities with 1 year of supervisory experience
- Ability to apply sound, mature decisions
- Willingness to work a diverse schedule based on the needs and preferences of the people we support
- Obtain and maintain a working cell phone
- Ability to bend, stoop, sit, stand, reach, and lift items weighing at least 75 pounds
- Meets 460 IAC 6 minimum qualifications for a direct care staff:
