Facilitator/ Case Manager

Job Summary
A-Alpha Resources, LLC is an LDH-licensed Personal Care and HCBS Waiver provider actively seeking a Facilitator / Service Coordinator to support individuals receiving Community Choice Waiver, NOW Waiver, and Personal Care Services. This role focuses on service coordination, case management, participant advocacy, and LDH compliance.

Responsibilities

• Coordinate and monitor Plans of Care (POC) in compliance with LDH and Medicaid HCBS regulations
• Provide case management and service coordination for waiver participants
• Conduct participant assessments, home visits, and ongoing monitoring
• Ensure participant health, safety, and welfare at all times
• Maintain accurate, timely documentation in LaSRS and agency systems
• Monitor service utilization, authorizations, and approved service hours
• Collaborate with Direct Support Workers, families, and interdisciplinary teams
• Identify changes in participant condition and initiate appropriate action
• Support quality assurance, audits, and corrective action plans
• Educate participants and families on rights, services, and program expectations

Requirements

• Bachelor's degree preferred in Social Work, Human Services, Psychology, Sociology, Nursing, or related field
• Minimum 1 year experience in:
• HCBS waiver programs
• Personal Care Services
• Case management or service coordination
• Knowledge of LDH regulations, Medicaid compliance, and Plans of Care
• Ability to meet documentation and compliance standards
• Strong organizational and communication skills
• Valid Louisiana driver's license and reliable transportation
• Must pass:
• Criminal background check
• Adult abuse registry check
• TB screening and health clearance (LDH requirement)

Preferred Qualifications

• Experience with LaSRS or Medicaid billing/documentation systems
• Prior experience with LDH audits, OCDD programs, or waiver services
• Familiarity with Community Choice Waiver or NOW Waiver

Pay: $ $25.00 per hour
