The Adolescent Treatment Services Coordinator position is part of the Child and Family Care Coordination Unit and employees in this role act as "Care Coordinators". Care Coordinators support children and families with care coordination, and link families to resources and services that are clinically necessary in a least restrictive environment to promote positive emotional and behavioral adjustment. Youth involved with Care Coordination have been identified as needing intensive mental health/behavioral health treatment and are referred to the unit for "Higher Level of Care" mental health services. These services are provided through service providers across a continuum of care ranging from community-based treatment to residential treatment. This position works closely with the family's formal supports (e.g., child welfare, juvenile justice, schools, etc.) and informal supports (e.g., neighbors, family members, etc.) to improve the academic, social, emotional, and behavioral outcomes for their child(ren).

- Participates in treatment planning and discharge planning in conjunction with the facility personnel as well as with members of other involved agencies to insure that the treatment results in the expeditious release of the adolescent patient.
- Designs the aftercare treatment plans and referring clients to appropriate outpatient treatment facilities, community support systems, and/or residential treatment facilities and insures that the discharge recommendations and aftercare plans are carried out.
- Interacts with clients' families interviewing members, observing family dynamics, assessing strengths or dysfunctions and adapts aftercare treatment plan to reflect their needs as well as those of the client.
- Provides direct supportive services including home visits, individual and family counseling as well as family crisis intervention.
- Monitors the outpatient treatment components to insure progress and resolve any problems that might arise.
