We are strategically onboarding well-organized individuals during slower seasons for a part-time role. This position is focused on long-term retention and development, offering flexible, occasional tasks with the potential to transition to a full-time role as client demand increases.
Minimum Requirements:
- Must reside in Minnesota. Be willing to travel and work from home and communicate virtually.
- Available for flexible, occasional tasks; hours fluctuate with client demand.
- Bachelor's Degree or 2 years of experience in providing direct social services (references required).
- Valid Driver's License and continual proof of current insurance coverage.
- Advanced computer skills (Word, Excel, Google Suite and other Internet databases).
- Strong independent work ethic, team collaboration, self-starter motivation, and flexibility.
- Ability to write and submit professional, detailed case notes daily via InSync, documenting all assigned tasks.
Key Responsibilities:
- Work as a Housing and/or Discharge Case Manager to provide Relocation Service Coordination Targeted Case Management (RSC-TCM) to assist clients with locating and moving to a safe and suitable home of their choosing that meets their needs.
- Provide Moving Home Minnesota (MHM) services to clients who qualify.
- Independently manage a caseload of clients that are needing to move from one home to another.
- Process assigned caseload based on priority level.
- Act as the client's advocate as it relates to completing tasks necessary to ensure successful completion of their relocation plan.
- Collect all documentation and information necessary to update the client's relocation plan .
- Complete forms and documents with clients.
- Send weekly caseload update to BLRS Management.
- Housing case management tasks will include conducting weekly housing searches for clients online and completing virtual and in-person tours as needed.
- Work collaboratively with other BLRS case managers, clients, families, physicians, social workers, and other professionals to ensure quality service is provided.
- Communicate with clients and everyone on the client's care team (service providers, family members, friends, etc) on a weekly basis regarding housing search leads and other needs including but not limited to: assessing client's care needs and wants, and providing community referrals for clients as needed.
- Collaborate with the client's other service providers and any relevant community agencies to assist the client with meeting their goals. Communicate effectively, positively and professionally with client's, providers, family members and others, often from diverse backgrounds.
- Attend weekly case consultation meetings.
- Coordinate clients housing search and discharge planning tasks, be present as needed on moving day.
- Submit case notes for all HSO tasks and DCO tasks daily.
- Collaborate with Independently Contracted Case Managers(HSOs) to ensure that tasks are being prioritized and that BLRS clients are receiving quality service.
- Upon successful completion of the initial one 3-4 week training period, meet a billable hour expectation of 15-25 hours per week.
- Report directly to both Team Lead regarding progress on client cases.
- Complete extra client related tasks as needed or as directed by Team Leads, management or administrative staff.
- Participate in virtual case reviews via Google Meet, to review client cases.
- Independently track progress of all cases with detailed notes.
- Write and submit professional case notes every shift and submit them on time via InSync.
- Maintain professionalism that is in accordance with BLRS philosophy and values.
- Adhere to HIPPA laws and regulations to ensure confidentiality of our client's files.
- Keep the BLRS management and BLRS Team informed of critical issues or factors which may impact your client's case.
- As needed, participate in development and implementation meetings with the BLRS management and BLRS Team.
- Explore opportunities to add value to your job role; find areas of personal improvement and suggest solutions for additional training.
- Participate in your own performance reviews with your manager.
- Other duties that may be assigned as the agency continues to grow.
- Ensure that BLRS distant clients have a client contact plan that will meet their needs during the intake process.
- Responsible for collaborating with our team to ensure that each client's initial documents and their tasks are up to date and reflect their current needs.

Job Details:
