Overview
We are seeking compassionate and dedicated Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs) to provide exceptional patient care across home health environments. The ideal candidates will have a strong background in patient observation, medication administration, and assisting with activities of daily living (ADLs). Experience with medical documentation, EMR/EHR systems, and working with individuals with developmental disabilities or dementia is highly valued. This role offers an opportunity to make a meaningful difference in the lives of patients while working in a supportive and professional environment.

Duties

  • Provide direct patient care including assistance with ADLs such as bathing, dressing, grooming, and mobility support
  • Monitor and document vital signs accurately using medical terminology and EMR/EHR systems
  • Assist with medication administration following HIPAA guidelines and care plans
  • Support patients with dementia, Alzheimer's, memory care, hospice needs, and long-term care requirements
  • Utilize specialized equipment such as Hoyer lifts and catheter care tools safely and effectively
  • Observe patients for changes in condition and report findings to nursing staff promptly
  • Assist with meal preparation, feeding, laundry, cleaning, and maintaining a safe environment
  • Document medical records thoroughly and accurately in compliance with healthcare regulations
  • Provide compassionate care tailored to the individual needs of patients with disabilities or in hospice settings
  • Work collaboratively within a team to ensure comprehensive patient care in assisted living or home & community care environments

Requirements

  • Valid CNA certification or HHA license required; CPR certification preferred
  • Prior experience in nursing homes, assisted living facilities, or home health care highly desirable
  • Knowledge of medical terminology, medication administration, and medical documentation standards
  • Experience working with people with developmental disabilities or dementia is a plus
  • Ability to operate EMR/EHR systems efficiently and maintain HIPAA compliance
  • Strong patient observation skills and the ability to assist with ADLs safely
  • Compassionate nature with excellent communication skills and patience when working with diverse populations
  • Ability to work independently or as part of a team in various settings including live-in employment options
  • Physical ability to assist with patient mobility tasks such as using Hoyer lifts or performing transfers
