Key Job Responsibilities: The Patient Services Coordinator coordinates care for TCT hospice patients and collaborates with the Clinical Manager and Intake Nurse to identify clinicians with the appropriate experience and skill set to meet patient needs.

Additionally, the Patient Services Coordinator will:

  • Assign Evaluation Visits
  • Process Rescheduled, Missed, and Reassigned Visits.
  • Reassign Declined Visits.
  • Complete Requested Schedule.
  • Run the Scheduling Requests Report several times a day.
  • Run the Client Schedule Report daily, several times per day.
  • Run Agent Summary Report.
  • Run the Visit Sut Report weekly.
  • Review Coordinator notes.
  • Verify paper visit notes.
  • Ensure all unscheduled visits are scheduled at least 7 days in advance, if possible.
  • Complete any additional duties as outlined in the PSC SOP.
  • Ensure compliance with agency policies and procedures for operations/processes, including, but not limited to, patient care, patient complaints, incidents, safety, and emergency management.
  • Always maintains patient confidentiality in accordance with HIPAA regulations.

  • Two years home care, hospice, scheduling, intake, discharge planning, or case management experience preferred.

  • Microsoft computer skill set required, experience with The Care Teams software and applications preferred.
  • Must be computer literate and ability to maintain simple records in English.
  • Excellent oral, written and analytical skills.
  • Should be a skilled organizer able to manage office files and logbooks.
  • Must possess light clerical skills and have a polite telephone manner.
