RESPONSIBILITEIS:
Successfully complete 90-day orientation.
Manage and categorize incident reports internal and external to create Risk Reduction Recommendations.
Upload and communicate monthly reports to individuals, employees, and individual's teams in regard to progress and recommendations to improve progress.
Assist in developing plans, strategies, and/or best practices for employees/contractors to improve levels of support to individuals.
Coordinate Annual Assessments including but not limited to
administration and collection of annual surveys to individuals, support teams, and employees.
Review documentation of progress of goals for each individual.
Create teams and coordinate completion of annual reviews.
Create recommendations based on annual review information.
Present information to leadership, employees and contractors.
Create reports based on incidents reports internal and external.
Review documentation content daily to ensure quality of content.
Assist Director of Operations with managing scheduling including but not limited to providing direct support to individuals as needed.
Assist in reviewing, implementing, and creating of policies, procedures, protocols, and/or rules.
Create training policies, procedures, protocols based on assessments, needs, and reports to improve quality.
Assist with training in all areas of Arrow Services Indiana to ensure quality of delivery.
Ensure quality of service delivery.
Make recommendations to improve quality, effectiveness of services, and satisfaction through means approved by the CEO/COO.
Review current policies and procedures and assist in implementing and enforcing policies and procedures.
Compile and analyze information from a variety of sources to create necessary reports.
Facilitate staff training.
Understanding of conducting training and needs assessment.
Assist in developing and implementing solutions for various challenges.
Responsible for ensuring timely submissions of required reports.
Assist in Investigations as needed.
Assist with on-call rotation.
Collaborate with all departments.
Coach Risk Reduction Direct Support Professionals in all areas of risk including reviewing documentation and creating plans.
Participate in annual trainings
Participate in annual surveys
Participate in annual assessments for the individuals served.
Participate and act as a member of the leadership team.
Participate with the risk reduction team via leadership team.
Participate, create, and assist with all areas of Quality Assurance and training via the leadership team.
Provide a minimum of 4 hours of direct support monthly.
Perform personal care tasks, including assistance with basic personal hygiene and grooming, feeding, and ambulation, medical monitoring, and health care related tasks. Fostering positive relationships between individuals served and their housemates
Ensuring individual safety and maintaining a safe environment
Assisting with toileting, including bedpans, urinals, and commode chairs
Reminding/assisting individual with self-administration of medications (with proper training)
Encouraging self-help activities
Reporting changes in individual's condition or family situation to the office
Documenting services provided
Performing home management functions such as light housekeeping, laundry, bed making, and cleaning
Planning meals, shopping for groceries, preparing and serving food/meals, feeding and clean-up
Accompanying individual(s) to scheduled appointments
Transporting individual(s) or running errands for individuals
Demonstrate ability to follow oral and written instructions
Implement the individual's support plan to reach their goals.
Teach and or/assist with daily living skills as necessary.
Effectively identify and respond to challenging behaviors outlined in the individual's Behavioral Support Plan.
Collaborate with the Director of Operations, Direct Support Staff, and Risk Reduction Direct Support Staff on community activities, for the individual's interest and needs.
Manage/address/assist with grievances. Assist other leadership members with investigations involving grievances.
Manage Open Door Policy reports for review.
Assist with review and development of policies, procedures, and protocols.
Responsible for reviews of individuals and company.
Manage individual's information at office to ensure information is up to date.
Job Type: Full-time
Pay: $35, $40,000.00 per year
Education:
- High school or equivalent (Preferred)
Experience:
- Management: 1 year (Preferred)
