Midland Care Connection
Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, it offers a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and its signature PACE (Program of All-Inclusive Care for the Elderly) program. PACE Program
The PACE program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Certified Medication Aide
Supports older adults by safely administering and documenting prescribed medications under the supervision of a licensed nurse. The CMA monitors participants for side effects or changes in condition, reports concerns to nursing staff, and helps ensure medications are taken correctly and on time. They may also assist with basic daily care related to medication routines, such as helping participants eat, monitoring basic observations, and providing comfort. As part of the interdisciplinary PACE team, the CMA communicates closely with nurses and other care providers to support coordinated, safe care that helps participants remain independent and live in the community.
QualificationsRequired
- High school diploma or GED
- Current Certified Medication Aide (CMA) certification
- Current CNA certification (if required by the program/state)
- Active CPR/First Aid certification or ability to obtain
- Ability to administer medications safely under nurse supervision
- Basic documentation and record-keeping skills
- Ability to follow care plans, policies, and procedures
- Reliable transportation and valid driver's license (if home or community visits are required)
- Ability to pass background check, health screening, and drug test
Preferred
- Experience working with older adults or geriatric populations
- Previous experience in a PACE program, long-term care, assisted living, or healthcare setting
- Knowledge of medication administration systems and electronic health records
- Strong communication and teamwork skills
- Compassionate, patient-centered approach to care
- Ability to recognize and report changes in participant condition
