Our primary care team is seeking a skilled and compassionate Advanced Practice Provider to join our dynamic practice and serve our growing community. Ideal candidates will possess excellent clinical, collaborative and communications skills, be comfortable using modern healthcare technologies and electronic medical records along with a strong commitment to serve our patient population. Position Overview: The ideal candidate will be experienced in providing comprehensive primary care services, focusing on preventive care, disease management, and overall health promotion for adult patients through geriatrics. As a Nurse Practitioner with us, you will have the opportunity to build long-term relationships with your patients while working in a supportive, team-oriented environment.
Key Responsibilities
- Provide comprehensive care to a diverse patient population, including adult, and geriatric patients.
- Virtually conduct thorough physical exams, diagnose medical conditions, and develop personalized treatment plans.
- Promote health education and disease prevention.
- Order and interpret diagnostic tests and manage chronic conditions.
- Collaborate with specialists and other healthcare providers to ensure coordinated care.
- Maintain accurate, up-to-date medical records and patient charts.
- Stay current with medical advancements and best practices in primary care.
Qualifications
- At least one year of direct clinical experience
- BSN or MSN required
- Active licensure in TN
- Completion of an accredited Nurse Practitioner program
- For Physician Assistants: Must be accredited by the American Medical Association
- Comfortable using new technology and software such as eClinicalWorks EMR
- Ability to work both independently and collaboratively within a team environment.
