Insight Disability Services is seeking a compassionate and dedicated Nurse Case Manager to join our team. This position plays a vital role in providing comprehensive healthcare oversight for individuals residing in our group homes while ensuring our staff remain in compliance with health and safety regulations. The Nurse Case Manager will conduct regular assessments of clients, coordinate necessary referrals for medical and therapeutic services, administer TB tests and health assessments for staff, and deliver a Medication Aide Course to employees.

Key Responsibilities

  • Client Care & Assessments
  • Perform regular physical, mental, and social assessments for individuals served in the group homes.
  • Develop, implement, and monitor individualized care plans in collaboration with the interdisciplinary team.
  • Provide recommendations and referrals for additional medical services, therapy, and rehabilitation as needed.
  • Ensure all nursing practices comply with relevant laws, regulations, and organizational policies.

Case Management & Coordination

  • Collaborate with healthcare professionals, caregivers, and outside providers to coordinate client care.
  • Maintain accurate and up-to-date documentation of all client care activities, interventions, and progress.
  • Serve as an advocate for clients, ensuring their needs are met and their health concerns are promptly addressed.

Staff Health Management

  • Educate and advise staff on infection control practices, personal protective equipment (PPE), and general health guidelines.

Quality Assurance & Compliance

  • Ensure adherence to all federal, state, and local regulations related to nursing, medication administration, and group home care.
  • Participate in audits and inspections, providing necessary documentation and follow-up as required.
  • Collaborate with leadership to develop and implement policies and procedures that enhance resident care and staff development.

Professional Development & Team Collaboration

  • Participate in ongoing training and continuing education to maintain nursing licensure and stay current with industry trends.
  • Collaborate with a multidisciplinary team, including administrators, social workers, therapists, and support staff, to provide holistic care.
  • Serve as a mentor and clinical resource for staff, fostering a culture of learning and continuous improvement.

Qualifications & Requirements

  • Education & Licensure:
  • Current Registered Nurse (RN) license in (State), in good standing.
  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) with relevant experience will be considered.
  • Experience:
  • Minimum of 2 years of nursing experience; experience in a community-based setting, group homes, or disability services strongly preferred.
  • Previous case management experience a plus.
  • Certifications:
  • CPR/First Aid Certification required (or willingness to obtain upon hire).
  • Certification in case management (e.g., CCM) is advantageous, but not required.
  • Skills & Competencies:
  • Strong organizational, communication, and interpersonal skills.
  • Ability to develop and deliver staff training programs.
  • Proficient in electronic medical records (EMR) and documentation.
  • Knowledge of state and federal regulations pertaining to healthcare in group home settings.
  • Compassionate approach with a passion for advocating for individuals with disabilities.

Physical & Working Conditions

  • Ability to travel between group homes within the Insight Disability Services network.
  • Must be able to perform standard nursing tasks, including lifting/positioning individuals or equipment as needed.
  • Work environment includes clinical, office, and residential settings, with occasional exposure to infectious diseases or bodily fluids.

Job Type: Full-time

Pay: $46.00 per hour
