Overview
We are seeking a compassionate and dedicated Hospice Nurse for On-Call to join our team.
Schedule is every other week
On-call week is Friday 5pm to Thursday 8:00am
Weekends are around the clock
Weekdays are 5pm to 8:00am
In this role, you will provide high-quality nursing care to patients with terminal illnesses, ensuring their comfort and dignity during their final stages of life. You will work closely with patients, families, and interdisciplinary teams to develop and implement personalized care plans that address the physical, emotional, and spiritual needs of those in your care.
Responsibilities
- Conduct comprehensive patient assessments to determine individual care needs.
- Administer medications and treatments as prescribed
- Provide emotional support and education to patients and their families regarding end-of-life care.
- Collaborate with physicians and other healthcare professionals to coordinate patient care.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Monitor vital signs, manage symptoms, and provide interventions for acute pain management.
- Implement infection control measures and maintain a safe environment for patients.

Experience
Candidates should possess a strong background in nursing with experience in hospice & palliative medicine or related fields. Preferred qualifications include:
- Familiarity with EMR/EHR systems
- Knowledge of ICD coding (ICD-9/ICD-10) and medical terminology.
- Background in geriatrics, long-term care, or senior care environments must have hospice experience.
- Skills in patient assessment, medication administration.
- Experience working with individuals with memory care needs is a plus.
- Additional experience in areas such as home care, outpatient services, or acute care is beneficial.
