Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have an opportunity for a Care Coordinator in Northside Chicago IL. We are looking for a dynamic individual to conduct comprehensive eligibility and needs assessments, facility pre-screenings, and authorizations for Community Care Program (CCP) services. This position ensures that all assigned clients receive timely, person-centered, and quality care coordination in alignment with state and agency standards.

Success in this role requires both professional competency and key interpersonal skills that support teamwork, communication, organization, and empathy for older adults and their families.

Key Functions include:

  • Conduct comprehensive Determination of Need/Eligibility assessments and complete all required documentation accurately and timely.
  • Develop individualized care plans based on assessment outcomes; ensure timely authorization and implementation of CCP services.
  • Enter all client data into state systems promptly and coordinate with support staff for service authorizations.
  • Review monthly contact updates, follow up on identified needs, and adjust care plans as necessary.
  • Maintain accurate and complete case records, including follow-up notes, reassessments, and service terminations.
  • Manage client grievances or service concerns with professionalism, empathy, and thorough documentation.
  • Participate in staff education, supervision, and team meetings to enhance care coordination quality and compliance

Qualifications:

  • Bachelor's degree in social science, social work, or a related field.
  • One year of relevant program experience (assessment, service authorization, or case management with the elderly) may replace one year of college education up to four years in lieu of a degree.

Experience:

  • Minimum of one year of experience in care coordination, client assessment, or case management preferred.
  • Experience working with older adults or individuals with long-term care needs strongly desired.
  • Comfort with Technology: Proficient in Microsoft Word, Excel, and state data entry systems; adaptable to new digital tools and platforms.

Travel:

  • 100% travel required: valid driver's license and satisfactory driving record necessary.
