JOB SUMMARY: Assists Solheim Senior Community ("Solheim") to the success of Solheim by effectively

assessing, planning, implementing, evaluating, and coordinating nursing services that

result in the provision of effective and high-quality care for our residents. Works in unison

with all other departments to ensure the highest degree of resident and family

satisfaction. Ensure staff compliance with Federal, State and local regulations. Ensure

staff compliance and participation in the promotion of the Mission, Vision and Wellness

philosophies.

ESSENTIAL JOB FUNCTIONS

Care Delivery Monitoring and Management:

  • Makes daily rounds andobtains report from the Charge Nurses and Certified Nursing Aides (CNA) and Restorative Nurse Aides (RNA) regarding resident change of condition and current resident clinical needs or concerns. Ensures staff performs shift to shift reports and follow narcotic management and accounting procedures.
  • Responds to all resident urgent or emergent situations. Provide clinical and administrative support to staff. Ensure that the Emergency Medical System (EMS) is activated in an appropriate and timely manner.
  • Monitors and observes daily care delivery; ordered treatments, management of pain, medication administration, medication storage management, as well as behavioral care and psychotropic medication use; for quality and compliance in keeping with community care standards, regulatory standards and Solheim policies/procedures.
  • Participates in medication administration and treatment delivery as needed.
  • Acts as role model, guide and facilitator for care needs and special concerns related to residents suffering from memory loss.
  • Participates in incident reporting and processing procedures. Assists staff in or performs resident physical assessment. Monitors, observes for and assesses resident personal safety needs. Consults with team. Ensures needed changes for preventive or reductive care are implemented, documented and communicated. Monitors and re-evaluates plan of care effectiveness. Performs environmental and room checks during rounds.
  • Reports concerns, condition or status changes to the Asst. Director of Nursing and/or the Director of Nursing Services. In the absence of the DNS; reports to the Skilled Nursing Administrator/Executive Director.
  • Demonstrates understanding of role and duties as a State Mandated Reporter for Abuse. Follows Abuse Prevention Protocols; participates in monitoring and identification, resident protection, investigations, reporting procedures, training and application of preventive measures.
  • Ensures documentation is accurate, timely and in accordance with policy and procedure. Performs chart and other audits as assigned.
  • Ensures physician orders are in compliance with policies and procedures; are complete, appropriate, and are accurately re-capped and reconciled. Audits and ensures MARs, logs, QA checks and forms are accurate, complete and done as assigned. Ensures appropriate destruction of discontinued medications is done as assigned.
  • Assist in providing telehealth services.
  • Ensures recommendations from the pharmacy consultant, behavior management team, weight variance committee, rehabilitation team and the Interdisciplinary Care Plan team are followed up on.
  • Responds to resident needs and requests promptly and appropriately. Acts as a "person centered" facilitator for the resident, responsible party, family, staff and other healthcare providers/vendors.
  • Follows infection control policies, procedures and standards of care. Provides guidance and monitoring for needed infection control and related procedures to the team. Reports concerns to the Asst. DON and/or DNS.
  • Monitors, coordinates and assists staff with new admissions, transfers and discharges. Ensures related policies and procedures are followed and documentation is complete. Ensures assessments are done in a timely manner. Provides therapeutic support and assists residents, responsible parties and families with explanations, education and information related to questions/issues/concerns. Work with the staff, Asst. DON and Medical Records staff to facilitate and ensure a smooth transition with intra-facility transfers.
  • Responsible for "First Responder" duties and performs initial assessment interaction with employees claiming to be injured on the job. Report employee related events to the Director, Human Resources before shift ends.
  • Prepares for and participates in regulatory inspections by the Department of Public Health, Licensing and Certification division and any other presenting entities.

Resident/Family/Guest/Staff Relations:

  • Reports issues of security, health and/or safety to the Director of Nursing Services, Executive Director or appropriate Department Manager as soon as practicable.
  • Ensures resident rights are preserved. Adhere to confidentiality, privacy and dignity expectations, including HIPAA rules.
  • Maintains effective listening and communication skills with residents, family, co-workers, physicians, ancillary and clinical service providers and vendors.
  • Attends to residents, family and responsible parties in a responsible and pro-active manner, addressing concerns and requests in a timely manner. Utilize active listening skills, positive body language and demonstrate a positive attitude.
  • Demonstrates good organizational skills, judgment, initiative, and problem-solving skills. Appropriately utilizes resources and acts as a resident advocate.

  • Effectively partners and communicates issues, needs and concerns to others; appropriately utilizes the chain of command. Provides support to all departments as the need arises.

  • Immediately reports concerns related to theft or loss through the appropriate chain of command.

Regulatory Compliance:

  • Follows established policies, procedures. Understands and complies with all regulatory standards set forth by governing entities.
  • Maintains Resident/Family confidentiality and privacy in accordance with HIPAA regulations, adheres to the Compliance Plan and policies and procedures.
  • Monitors staff compliance. Reports concern regarding compliance to the Director of Staff Development and/or the ADON for further action.

Administrative Compliance:

  • Adheres to payroll policies and assures direct report(s) properly uses timekeeping system with minimal manual changes.
  • Maintains regular and consistent attendance.
  • Maintains compliance with employee handbook rules. Reports concerns regarding staff compliance to the Director of Staff Development and/or Asst. Director of Nursing.
  • Performs frequent rounds. Provides coaching, organizational advice and educational information to employees regarding daily care, skills, procedures and tasks. Reports concerns to the Director of Staff Development and/or the ADON for further action.
  • Adheres to Department of Public Health OBRA and Title 22 rules and regulations, Solheim Compliance Plan, HIPAA and other related regulations. Monitors, staff compliance. Provides coaching and educational information to employees as needed. Reports concerns regarding staff compliance to the Director of Staff Development and/or ADON.
  • Maintains compliance with personal protective equipment use and safety procedures. Monitors, staff compliance. Provides coaching and educational information to employees as needed. Reports concerns regarding staff compliance to the Director of Staff Development and/or ADON.
  • Assists Human Resources to administer the Workers' Compensation Program by acting as the "First Responder Nurse." Provides an initial assessment, First Aid care (if needed) and assists the employee to determine the need for additional medical treatment.
  • Performs other duties as assigned.

POSITION REQUIREMENTS:

EDUCATION/EXPERIENCE:

  • High school graduate or equivalent.
  • Bachelor's degree preferred.
  • Graduate of an accredited nursing school and currently licensed as an RN in the State of California.
  • Minimum of two years' experience in a Long-Term Care Facility or equivalent experience.
  • Proficient in using computers.
