Compensation and ScheduleStatus: Full-time, $17.41 hourly

Aptiv is seeking a Service Navigator to support participants and their families in receiving person-centered services.
In this role, you'll provide a blend of direct support and case management, helping individuals with disabilities live more independent, fulfilling lives. You'll work closely with families, funders, and the Aptiv team to ensure services meet participants' needs — advocating for health, safety, and independence every step of the way.

ResponsibilitiesAs a Service Navigator, you will:
Provide direct and indirect support to Aptiv participants within assigned departments
Build relationships with participants and families to ensure their services reflect personal goals and preferences
Conduct intake, assessments, and regular plan meetings for individuals on your caseload
Maintain accurate and up-to-date documentation and reports in Aptiv's database (SETWorks)
Advocate for participants by identifying service needs, requesting appropriate authorizations, and ensuring services are aligned with health and independence goals
Collaborate with program managers, supervisors, and team members to ensure high-quality, person-centered services
Encourage community integration and help participants build natural supports
Monitor authorizations and ensure services are accurately scheduled and billed
Maintain compliance with all Aptiv policies, procedures, and regulatory standards
Participate in team meetings, maintain confidentiality, and uphold high ethical standards
Preferred:
Bachelor's degree in a human services field such as Sociology, Psychology, Education, Recreation Therapy, or a related area
3 years of experience working in a human services setting
Experience with case management, service authorizations, and supporting individuals with disabilities
Strong communication, organization, and documentation skills
Knowledge of proactive behavioral approaches and precision teaching techniques
Required:
Valid driver's license and reliable transportation with current insurance
Ability to operate computer equipment and use Microsoft Office and Google applications
Flexibility in scheduling and time management
Ability to provide direct support to individuals served
