Responsibilities
- Conduct comprehensive evaluations of patients' physical conditions by applying physiology and anatomy knowledge to identify impairments and functional limitations.
- Develop and execute tailored care plans that incorporate evidence-based physiotherapy techniques such as trigger point therapy and orthopedics interventions.
- Deliver hands-on therapy sessions focusing on improving mobility, strength, and flexibility across diverse populations including pediatrics, geriatrics, and post-surgical patients.
- Collaborate with multidisciplinary teams to coordinate discharge planning, ensuring seamless transitions from hospital to home or community care settings.
- Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes in inpatient, outpatient, or home health environments.
- Maintain accurate documentation using electronic medical record (EMR) systems to record assessments, treatment plans, progress notes, and discharge summaries.
- Educate patients and their families on injury prevention, physiotherapy exercises, and self-care techniques to promote long-term wellness.
Requirements
- Valid licensure or registration as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience in acute care hospital settings, outpatient clinics, or home health environments with familiarity in inpatient and community-based care.
- Strong knowledge of physiology, anatomy, medical terminology, and specialized therapies such as trigger point therapy.
- Experience with care planning for diverse populations including pediatrics and geriatrics; familiarity with orthopedics and sports injury rehabilitation is preferred.
- Proficiency in EMR systems for documentation and patient management; ability to adapt quickly to new technology platforms.
- Excellent patient care skills with a compassionate approach to treatment delivery; ability to work independently and as part of a team.
- Knowledge of discharge planning processes and experience coordinating care across multiple settings.
