Expert Community Care Management (ECCM) is a non-profit organization focused on helping individuals navigate complex systems to access necessary services. Through programs such as early intervention service coordination, autism supports coordination, intellectual disabilities supports coordination, and behavioral/mental health case management, ECCM empowers individuals to achieve greater independence. Role Description

The Supports Coordinator role is a full-time, hybrid remote position based in Lancaster, PA. This role involves providing service coordination and case management for individuals with unique needs. Responsibilities include engaging with consumers to assess their support needs, connecting them with appropriate resources, facilitating service planning, maintaining accurate documentation, and collaborating with service providers and community agencies. The position requires effective support for participants and their families to ensure access to the benefits and services they are eligible for, as well as promoting their overall independence and well-being.

Qualifications

  • Strong Interpersonal Skills and the ability to build meaningful relationships with individuals and families.
  • Excellent Communication skills, including verbal and written proficiency, for interacting with diverse teams and stakeholders.
  • Experience with Case Management and the ability to develop and monitor service plans effectively.
  • Proficiency in Customer Service and the capacity to address the unique needs and concerns of consumers in a professional manner.
  • Experience in providing Technical Support or a willingness to learn technical tools related to case management systems.
  • Bachelor's or Associates degree in social work, psychology, or a related field, or equivalent relevant experience.
  • Knowledge of community resources and services is an asset.
  • Ability to work independently and manage time effectively in a team-oriented environment.
