Position Summary:

The Unit Coordinator oversee the administration of nursing care to designated residents under the direction of nursing leadership. Coordinates and supports the activities of the certified nurse assistants and may actively participates in the direct care of the resident, by following the specific approaches as stated in the plan of care.

  • Plans effective administration of nursing services in cooperation with other health related services on their unit, in compliance with nursing services philosophy and objectives.
  • Responsible for assessment of resident needs, development and implementation of comprehensive care plan. Consults with resident physicians in planning resident care. Periodically reviews and updates comprehensive care plan.
  • Supervises planning of daily assignments for personnel on the unit.
  • Conforms with established policies and procedures and maintains staff awareness of these in their daily work.
  • Schedules and leads staff conferences to promote initiative and solve resident care-oriented problems.
  • Works with health team members in developing a discharge plan for individual residents and assists in achieving discharge planning.
  • Interprets principles of good nursing care and management to nursing personnel and implements application of these principles in daily work in cooperation with other resident service personnel.
  • Evaluates incidents and determines emergency measures to be used, including the use of restraints. Examines and may give direct emergency treatment to residents and personnel involved in accidents. If necessary, notify the physician. Completes the incident report with causes investigated and corrective action taken and/or suitable recommendations made to avoid recurrence.
  • Initiates and completes Care Plan
  • Assists in orienting and directing new personnel in the Nursing Department.
  • Reviews and interprets work performance against accepted standards. Responsible for completion of performance evaluations of 7-3 nursing staff on his/her unit. May review and provide input for performance evaluations for nursing staff on alternate shifts.
  • Participates in dermal, weight, dietary, etc. meetings
  • Directs requisition of equipment and supplies and encourages proper utilization of same.
  • May provide input to the Inservice Coordinator in planning nursing in-service programs and on-the-unit conferences based upon an assessment of staff's learning needs and/or resident care problems.
  • Handles resident or personnel grievances and/or refers them to the proper department head.
  • Assists in establishing and maintaining good personnel and public relations.
  • Maintains accurate, adequate records of resident care and related statistics as required.
  • Is available for emergencies and assistance to nursing personnel as needed.
  • Communicates and acts as a liaison between resident, family, physician and staff.
  • Accompanies physician on rounds, examinations and treatments.
  • Incorporates the philosophy, objectives, policies and standards of the Home in correlation with resident care.
  • Promotes and maintains safe work performance by employees.
  • Informs the nursing supervisor and assistant director of nursing services of pertinent information concerning resident status, personnel actions, employee attitudes and suggestions.
  • Conducts resident rounds to assess and/or supervise nursing care, ascertain condition of residents and safe, clean, comfortable environment.
  • Receives reports from personnel on previous tour of duty and ensures that the continuity of care continues by personnel on ensuing tour.
  • Participates in Nursing Management Meetings and Comprehensive Care Plan Conferences.
  • Participates in formulating nursing policies and procedures.
  • Notifies the resident's physician, family and/or power of attorney of any change in the resident's condition.
  • Continues professional growth through reading current professional literature and attending professional meetings, seminars, and workshops.
  • May perform duties listed on position description for Team Leader in his/her absence.
  • Performs other duties as required.
  • Administers IV medications, orders flushes and care in absence of staff RN.
  • Always adheres to dress code policy.
  • Always adheres to time and attendance policy.
  • Practices safe work habits and follows all safety procedures of the facility.

Qualifications

Position Requirements - Unit Coordinator

EXPERIENCE: Experience preferred in geriatric, medical and surgical nursing.

EDUCATION REQUIREMENTS: Graduate of an approved school of nursing. Current registration in New York State is required. LPN's must have CPR training.  If an LPN, candidate must be currently working towards obtaining an RN License.

SPECIFIC REQUIREMENTS:

  • Ability to cooperate with other personnel and to supervise when necessary.

PHYSICAL AND SENSORY REQUIREMENTS:

  • Must have the physical ability to lift a minimum of 50 pounds.
  • Must submit to and pass a yearly examination, PPD skin test and attend all mandatory in-services.
  • Must have the ability to push, pull and bend throughout the day.

WORKING CONDITIONS:

  • Stands, sits and walks most of shift.
  • Subject to many interruptions and changes of priorities.
  • Periodic contact with residents, employees, and families.
  • Required to make decisions within the established guidelines.
  • Required to handle emergency situations.
  • Well-lit, well-ventilated nurses' stations, resident rooms and halls.
  • Exposed to infectious odors, peculiarities, and mood changes of aging persons.
  • Frequent heavy lifting and moving of residents.
  • Must share weekend and holiday work.
