Position Description

Advertised Job Summary

Community Health Worker will work with licensed research nurse (RN) and other team team members to assist in coordinating and delivering a telehealth/remote intervention with patients with chronic health conditions, as well as their Carepartners, for a research study funded by the National Institutes of Health (NIH) that seeks to improve current and future chronic illness management among adults and their caregivers (Carepartners) in medically underserved and rural areas of South Carolina. Work collaboratively and efficiently with the research team to deliver the intervention component of the study in accordance with the protocol, study procedures, and established timelines. Ability to work with a level of independence and also in concert with the research team, patients, and their respective care partners to operationalize the devised wellness goals/plans by providing resource navigation, coordination/referral assistance, and facilitating access to care and other community resources.

Qualifications

  • Experience working with under-resourced communities, rural and/ or historically marginalized adult populations with chronic health conditions.
  • Experience with collaborative community-based rural health research or clinical trials.
  • Experience with Microsoft Office applications, including Word, Excel, PowerPoint, Outlook, and Teams.
  • Excellent interpersonal and communication skills.
  • Ability to multitask a variety of functions.
  • Organized, self-motivated, and detail-oriented.
  • Must be able to work with limited supervision.
  • Flexible schedule to be pre-arranged with the supervising team leader.
  • Some evenings/weekends.
