Tabor Hills Senior Living Campus, a not-for-profit community located in Naperville, IL, has been dedicated to caring for our senior population for 130 years. Tabor Hills offers an Independent Living Community, Supportive Living Community and a Skilled Nursing Center.

We have a rare opportunity for a Social Service Associate to join our team at our healthcare building.

This is a part time position: Tuesdays, Wednesdays, Thursdays 9:30 a.m. to 6:00 p.m.

Highlight of Responsibilities:

  • Under the supervision of Director of Social Services will complete necessary assessments including MDS, cognitive assessments, documentation in progress notes, social history, care plan, prepare Social Services Admission paperwork and complete with Resident/Families.
  • Fulfill additional responsibilities that relate to the psychosocial well- being of the resident population.
  • Assists with facilitation of resident room moves by contacting families and talking to the resident.
  • Assists Admissions with tours and contracts as required.
  • Other duties as assigned.

Qualifications:

  • Education: Bachelor's degree in human services or related field.
  • Experience: One year experience with geriatric and/or long-term care facility preferred.
