Responsibilities

  • Monitor patient health by checking vital signs, observing changes in condition, and maintaining accurate documentation in compliance with HIPAA regulations
  • Prepare nutritious meals tailored to individual dietary needs and assist with feeding as required to promote proper nutrition and hydration
  • Administer medications following care plans and ensure proper documentation of medication administration
  • Support residents with cognitive impairments such as Alzheimer's disease or dementia through memory care techniques and engaging activities
  • Maintain a clean and safe environment by performing light cleaning duties, laundry, and ensuring infection control protocols are followed
  • Collaborate with healthcare professionals to develop and implement personalized care plans that address each resident's unique needs and preferences

Requirements

  • Prior experience in home & community care, assisted living, nursing homes, or group homes is highly desirable
  • Knowledge of patient observation techniques, vital signs monitoring, and basic nursing skills including CPR and First Aid certification
  • Experience working with individuals with developmental disabilities, autism spectrum disorder, or dementia is preferred
  • Ability to lift heavy objects safely and assist residents with mobility using appropriate equipment such as Hoyer lifts or resident's lifts
  • Strong communication skills to effectively interact with residents, families, and healthcare teams while maintaining confidentiality per HIPAA standards
  • Compassionate attitude combined with patience and resilience to handle challenging situations calmly and professionally
  • Flexibility to work various shifts including evenings, weekends, or holidays as needed for comprehensive coverage
