A growing healthcare facility in Fresh Meadows is seeking a Licensed Practical Nurse (LPN) to support both telehealth and infusion services. This is a full-time role (Monday-Friday, 9 AM-5 PM) with growth potential into clinical infusion care.
Key Responsibilities:
- Start off managing patient coordination and telehealth workflows
- Conduct telehealth intake and assist providers with virtual visits
- Support patients with establishing care, referrals, and follow-ups
- Document visits accurately and maintain clear communication with the care team
- Once infusion volume increases, transition into hands-on infusion therapy (training provided if needed)
- Current NYS LPN license
- Experience with telehealth preferred
- Willingness to transition into infusion care as the role evolves
- Proficiency in Russian is preferred
- Strong communication, organization, and EMR documentation skills
