JOB TITLE: Supports Coordinator Assistant
LOCATION: Cheboygan, MI 49721
REPORTS TO: Center Supervisor
STATUS: Non-Union, Full-Time, Non-Exempt
HYBRID ELIGIBLE: Community-Based Position
STARTING WAGE RANGE: $ $20.50 per hour

QUALIFICATIONS:
Education: Minimum of high school diploma or equivalent required. Credit given for relevant life experience. Some college preferred, especially courses in social work, education, psychology, nursing, speech pathology, recreation, occupational therapy, physical therapy, human services or other related field.
Experience: One-year experience in working with persons with developmental disabilities is preferred but other combinations of experience will be considered
Other: Lived experiences with behavioral health issues are valued.
SUMMARY OF RESPONSIBILITIES: This is an entry level, paraprofessional position providing a variety of supports to individuals with developmental disabilities. Individuals may live in their own homes, with family members, or in specialized or non-specialized dependent care settings. All supports coordination services are provided in accordance with state and federal regulation including, but not limited to, the Michigan Mental Health Code, Medicaid standards, especially those related to the Habilitation Supports Waiver, ICF/MR, and Clinical Services.
The Supports Coordinator Assistant performs the "customary" tasks of supports coordination, under the immediate supervision of a degreed supports coordinator and the final supervision of the support services supervisor. Supports coordination includes the traditional case management functions of assessment, support plan development, monitoring, linking and coordination of supports. In addition, supports coordinators help with individual empowerment issues, individual health and safety, mobilization of natural supports, facilitate access to community resources, and help the individual achieve the goals or supports as determined through a person-centered planning process. Community inclusion, normalization, personal choice, self-determination, natural supports and empowerment are key principles.
The Supports Coordinator Assistant position will include the provision of supportive services to families who are allocated respite only though the agency and be an alternate liaison with DCH for Family Support Subsidy
ESSENTIAL JOB FUNCTIONS:
May assist the Supports Coordinator in the development of Plans of Service for specific individuals. Duties may include gathering data, completing forms, taking notes and writing up minutes.
May assist the Supports Coordinator in linking individuals with community and/or family resources. Duties may include transportation to appointments, home visits, shopping, etc.
May assist the Supports Coordinator in the completion of monthly and quarterly progress reports and status reviews as required. May help gather and summarize data required for these reports.
May assist the Supports Coordinator in monitoring and assuring that applicable laws, rules, policies and procedures are followed in the development and implementation of each consumer's Plan of Service.
May assist the Supports Coordinator, individual and/or parents/guardians in making application for all necessary entitlement benefits, financial and medical assistance, residential support funding.
Attend meetings as needed or requested by the Supports Coordinator or supervisor.
Assist as needed in transporting and accompanying individuals to medical appointments, evaluations, etc. Help individual and/or caregivers/parents/guardians in interactions with community service providers (health care professionals, employers and social service agencies). Irregular hours may be necessary periodically.
May assist the Supports Coordinator in filing guardianship papers, preparing for guardianship hearings.
May assist the Supports Coordinator in completing original or updated psychosocial assessments for assigned individuals. Psychosocial assessments include psychosocial functioning, health, educational and vocational functioning, with recommendations for treatment planning/intervention.
Provide complete and timely documentation of relevant activities in the paper or electronic clinical record and the agency information management system in accordance with agency policy and procedure and Medicaid requirements.
Meet direct and indirect service activity expectations.
Perform all other duties as assigned by supervisor.
PHYSICAL REQUIREMENTS:
Ability to walk, bend, stand, sit, lift to 50 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive a vehicle. Position requires physical ability to complete and maintain certification in de-escalation techniques which will be a training class provided to the job incumbent. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions in accordance with applicable laws.
OTHER REQUIREMENTS:
- Must be current in all applicable trainings; provided by NCCMH.
- Must have own phone and internet for communication, reliable access to messages, and multi-factoring capabilities.
- Have reliable transportation in carrying out agency duties.
- Must be an actively participant of a team setting and willing to offer assistance to others when needed.
- Possess a valid Michigan's driver's license and have a satisfactory driving record.
- Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies.
- Supports a philosophy of service delivery that is recovery-based, person-centered and culturally competent.
- Adhere to applicable policies and procedures as required by the Department of Community Mental Health, the Mental Health Services Board, and other pertinent regulatory agencies.
- Must always maintain confidentiality regarding consumer data.
