Job Summary

Physical Therapist ("PT") to provide quality care to our patients. The PT evaluates and treats patients, communicates, and provides education to patients/their families, and maintains all necessary documentation of services in the patient's medical records. Initiates discharge planning for physical therapy. The PT implements the treatment plan through direct treatment of the patient, delegating a portion of the treatment plan to appropriate supportive personnel, and coordinates the treatment program with other health care personnel as appropriate. The PT will also analyze rehabilitation goals and collaborate with the physician and rehab team in order to coordinate the most effective physical therapy program.

Responsibilities and Duties

Essential Responsibilities and Expectations

  • Evaluate patients promptly, and per facility policies and expectations, upon receiving physician referral for treatment
  • Develop effective treatment plans
  • Obtain approval for services from referring physician
  • Treat patients according to treatment plan approved by physician
  • Communicate regularly with supervisor, other members of the rehabilitation team, and family regarding patient treatment plan, progress, prognosis, and planned discharge
  • Record evaluations, daily treatment notes, re-assessments, and discharge summaries per Company policy and procedure
  • Complete all required documentation from your daily treatments/interventions prior to leaving the facility
  • Educate patient families and caregivers in treatment and any applicable maintenance programs
  • Participate in discharge planning
  • Secure necessary durable medical equipment (e.g., orthoses, walkers, canes, standers, wheelchairs, etc.) for patients to facilitate independence
  • Submit all billing and necessary information prior to leaving the facility
  • Supervise Physical Therapist Licensed Applicant (PTLA), supervise licensed Physical Therapy Assistants ("PTA"), and supervise a Rehabilitation Aide ("RA") appropriately
  • Report any problem areas/equipment within each facility to the appropriate supervisor immediately
  • Comply with State Licensing Board of California, Title 22, CMS, and California Labor Board guidelines
  • Comply with Company rules, policies and procedures
  • Comply with all applicable safety rules, policies, and procedures
  • Report to work regularly and on-time

Additional Responsibilities and Expectations

  • Be willing and able to act as a mentor for new or recently hired PT/PTA
  • Provide necessary training and guidance to student interns as assigned, including:
  • Supervise students regularly
  • Providing clearly defined, career-related (i.e., non-clerical) responsibilities and tasks in accordance with the students' academic programs
  • Informing student interns about and ensuring their compliance with Company, facility, state, and federal guidelines
  • Completing any paperwork required by the Company or the students' academic program
