The Legacy at Home Social Worker provides social services under the direction of a physician, based on the patient's psychosocial assessment and the needs of the patient and their family. They will coordinate care with the Interdisciplinary Group (IDG) and participate in the on-call rotation as needed.
- Assess hospice patients to identify psychosocial, financial, environmental, and community needs.
- Provide counseling and support to patients and families coping with terminal illness.
- Use effective interpersonal and communication skills to facilitate care coordination with the interdisciplinary team.
- Participate in interdisciplinary team meetings and the patient care planning process.
- Maintain a working knowledge of public and private eligibility standards for patient resources.
- Meet continuing education requirements as mandated by the agency/licensing board.
- Promote agency philosophy and policies to ensure high-quality patient care.
- Accept clinical assignments aligned with education and competencies to meet patient needs.
Educational Requirements and Experience:
- Bachelor's Degree in Social Work from a State-accredited program.
- Texas Certification required.
- Current Driver's License required.
- At least one (1) year of social work experience in a healthcare setting.
- Strong knowledge of community resources and services.
- Excellent interpersonal and communication skills.
- Reliable transportation required.
