Job Summary

In this vital role, you will provide personalized assistance to promote independence, safety, and well-being in various settings such as home care, assisted living facilities, or group homes. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patients' health status, noting vital signs and behavioral changes to report to healthcare professionals
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Support medication administration following care plans and HIPAA confidentiality standards
- Operate equipment such as Hoyer lifts or wheelchairs safely to assist with patient transfers and mobility
- Maintain a clean, safe environment by performing light housekeeping tasks like cleaning patient areas and laundry
- Document care activities accurately in compliance with healthcare regulations and facility policies
Skills
- Proven experience in home health, assisted living, nursing homes, or group homes with a focus on senior or disability care
- Knowledge of patient observation techniques, vital signs monitoring, and medication administration
- Familiarity with specialized care such as Alzheimer's/dementia care, memory care, hospice support, or working with individuals with developmental disabilities or autism
- Certifications in CPR and First Aid are highly desirable; additional training in HIPAA compliance is preferred
- Ability to lift heavy objects safely and operate assistive devices like Hoyer lifts or resident lifts
- Strong communication skills for working effectively with patients, families, and healthcare teams
- Compassionate attitude coupled with excellent organizational skills for managing multiple care responsibilities
