The Physical Therapy Program Coordinator (must be a licensed Physical Therapist in Pennsylvania) will be responsible for leading the start-up, development, and ongoing coordination of a new in-home and community-based physical therapy program designed for individuals with intellectual and developmental disabilities (IDD) .
This role will be instrumental in building the program from the ground up — designing workflows, establishing best practices, and shaping a model of care that is both innovative and person-centered .
In addition to program development, the coordinator will oversee daily operations, guide staff growth, and collaborate with an interdisciplinary team to deliver exceptional outcomes. This position balances direct clinical care with leadership responsibilities, ensuring that therapy services remain effective, individualized, and compliant with the highest regulatory and quality standards. The coordinator will serve as both a program builder and clinical leader providing vision, structure, and mentorship to ensure the long-term success of the program.
Key Responsibilities:
A. Program Oversight and Growth
Design, launch, and manage a physical therapy program tailored to individuals with IDD.
Oversee daily operations, ensuring services are efficient, effective, and individual-centered.
Develop and maintain program policies and best practices that reflect quality care standards.
Partner with leadership to establish workflows, referral systems, and outcome measures.
Support staff hiring, onboarding, and training as the program expands.
Manage scheduling to align therapy resources with individual needs.
B. Leadership and Team Development
Supervise and mentor physical therapists, fostering clinical growth and professional excellence.
Provide feedback, coaching, and performance evaluations to ensure high-quality service delivery.
Lead team meetings to promote collaboration, innovation, and consistent communication.
C. Clinical Excellence
Conduct assessments and develop individualized treatment plans.
Deliver direct physical therapy services as needed, including in-home visits.
Collaborate with clinicians, caregivers, and support staff for coordinated care.
Educate families and caregivers on techniques to support progress outside therapy.
D. Program Quality and Evaluation
Monitor treatment outcomes and adjust care plans as needed.
Act as a resource for complex cases, offering clinical expertise and guidance.
Collect and analyze program data to drive continuous improvement.
Ensure compliance with regulatory, licensing, and quality standards.
E. Collaboration and Communication
Work closely with OTs, SLPs, nurses, and physicians to deliver holistic, interdisciplinary care.
Serve as the primary contact for families, keeping them engaged and informed.
Provide ongoing training to staff and caregivers to ensure progress continues beyondv therapy sessions.
F. Compliance
Ensure adherence to all state and federal regulations governing physical therapy practice, including documentation and billing standards.
Maintain up-to-date knowledge of Medicaid, waiver, and insurance guidelines relevant to IDD services
Uphold ethical and professional standards in all clinical and administrative functions.
Ensure staff compliance with safety protocols, HIPAA regulations, and OneWell Health Care policies.
Participate in internal audits, quality reviews, and corrective action plans as needed to maintain program integrity.
Qualifications:
A. Education and Licensure
Doctor of Physical Therapy (DPT) or Master's degree from an accredited program.
Current Pennsylvania Physical Therapist license.
Successful completion of the National Physical Therapy Examination (NPTE).
B. Experience
At least 2 years as a licensed PT with success delivering individualized care.
Supervisory or leadership experience preferred.
Knowledge of treatment planning, quality assurance, and compliance requirements.

D. Competency Requirements
Strong ability to design and implement individualized physical therapy interventions while leading program development and oversight.
In-depth knowledge of developmental disabilities and their impact on movement, coordination, and functional independence.
Excellent communication and collaboration skills for working with interdisciplinary teams, families, and caregivers.
Demonstrated leadership in mentoring and supporting physical therapists and clinical staff.
Skilled in the use of adaptive equipment, mobility aids, and assistive technology relevant to individuals with IDD.
Strong analytical and organizational abilities for managing schedules, evaluating program outcomes, and maintaining compliance.
Empathy, patience, and adaptability to meet the diverse physical and emotional needs of each individual served.
Reports to: Director of Rehabilitation and Therapy Services
Work Environment:
Full-time, Monday–Friday, 8:30 AM–5:00 PM.
Primarily office-based with occasional travel for in-home evaluations, meetings, and community visits.
