Care Manager

Job Overview
We are seeking a dedicated and compassionate Care Manager to join our team. The ideal candidate will play a crucial role in providing support and guidance to individuals facing various challenges, including behavioral health issues, addiction, and social welfare needs. This position requires strong crisis management skills, a background in social work The Care Manager will work closely with clients to develop personalized care plans that promote recovery and well-being.

Duties

• Conduct comprehensive assessments of clients' needs, strengths, and challenges.
• Develop and implement individualized care plans in collaboration with clients and their families.
• Provide crisis intervention and management to clients experiencing acute situations.
• Facilitate access to community resources, including healthcare and mental health services, housing assistance and
• Collaborate with multidisciplinary teams to coordinate care for clients across various settings.
• Maintain accurate documentation of client progress and interventions in compliance with regulatory standards.
• Advocate for clients' rights and needs within the healthcare system.

Requirements

• Bachelor's degree in Social Work, Psychology, or a related field.
• Proven experience in behavioral health, crisis intervention, or related fields.
• Familiarity with motivational interviewing techniques and their application in client interactions.
• Experience in childcare or child welfare services is highly desirable.
• Knowledge of Medicare regulations and community resources available for clients.
• Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
