Job Description:

PURPOSE:

The primary purpose of the Licensed Practical Nurse Assessment Coordinator position is to conduct, coordinate, and ensure the accurate development and completion of resident assessments in accordance with state regulations, federal requirements, and the policies and goals of the facility. The Licensed Practical Nurse Assessment Coordinator plays a critical role in maintaining regulatory compliance, supporting interdisciplinary collaboration, and ensuring resident-centered care planning.

Duties and Responsibilities
  • Conduct and coordinate the development and completion of resident assessments (MDS) in compliance with current state, federal, and facility regulations.
  • Serve as the Licensed Practical Nurse Assessment Coordinator responsible for the accuracy, timeliness, and transmission of all required assessment data.
  • Implement and monitor RAPs, triggers, care plans, and assessment schedules.
  • Maintain and update policies and procedures governing resident assessments and care planning.
  • Ensure initial, quarterly, annual, significant change, and discharge assessments are completed within required timeframes.
  • Act as chairperson and active participant of the Interdisciplinary Care Plan Team.
  • Coordinate interdisciplinary input to develop comprehensive, resident-centered plans of care.
  • Monitor Quality Indicator (QI) and Quality Measure (QM) reports and implement corrective actions as needed.
  • Participate in facility surveys, audits, and inspections conducted by regulatory agencies.
  • Maintain confidentiality of all resident health information and ensure HIPAA compliance.
  • Assist with discharge planning and ensure care plans reflect resident goals, preferences, and advance directives.
  • Provide training and education to staff regarding MDS completion and assessment accuracy.
  • Report assessment-related concerns, trends, or deficiencies to the Administrator and Director of Nursing Services.
  • Ensure secure use of electronic systems, user IDs, and passwords related to resident information.
  • The Licensed Practical Nurse Assessment Coordinator ensures care delivery aligns with resident rights, dignity, cultural preferences, and individualized needs.

Skills and Qualifications
  • Must possess a current, active, unencumbered Licensed Practical Nurse (LPN) license in the state of PA.
  • Nursing degree from an accredited college or university.
  • Minimum of two (2) years of supervisory experience in a hospital, skilled nursing facility, or healthcare setting.
  • Minimum of six (6) months of experience or training in rehabilitative and restorative nursing practices.
  • Demonstrated knowledge of MDS, care planning, and resident assessment processes.
  • Strong understanding of age-specific care needs for the geriatric population.
  • Ability to communicate effectively with residents, families, interdisciplinary team members, physicians, and regulatory agencies.
  • Proficient computer skills and experience with electronic health records.
  • Ability to work independently, prioritize tasks, and meet strict regulatory deadlines.
  • Must read, write, speak, and understand English fluently.
  • Ability to lift, push, pull, and carry up to 25 pounds and assist with resident evacuation during emergencies.
  • Willingness to perform duties involving exposure to blood and bodily fluids while following safety protocols.
  • The Licensed Practical Nurse Assessment Coordinator must demonstrate professionalism, integrity, teamwork, and accountability at all times.
