'Clapp's Nursing Home is seeking a dedicated Licensed Practical Nurse to join our team. This vital role involves providing exceptional patient care within our specialized rehabilitation environment, supporting our mission to facilitate quick and comfortable recoveries for our patients. This position offers an opportunity to make a meaningful difference in the lives of those living in long term care.
*Responsibilities*
* Deliver high-quality nursing care to patients in a long term care setting, ensuring safety and comfort.
* Administer medications and treatments accurately according to physician orders.
* Monitor patient progress and document findings meticulously using electronic health records.
* Collaborate with physicians, therapists, and other healthcare professionals to develop personalized care plans.
* Respond promptly to patient needs and emergencies, providing compassionate support.
* Maintain compliance with healthcare regulations and safety standards.
* Participate in ongoing training to stay current with best practices in nursing care.
*Qualifications*
* Valid Licensed Practical Nurse (LPN) license.
* Proven experience in critical care, hospital, or long-term care settings.
* Proficiency with electronic health record systems such as Point Click Care
* Excellent communication skills and ability to work collaboratively within a multidisciplinary team.
* Compassionate approach with a focus on patient-centered care.
* Ability to handle multiple priorities efficiently.
* Preferred: Experience working with elderly or disabled populations, including memory or Alzheimer's care.
