Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Dayshift team. The ideal candidate will have a strong background in patient care across various settings, including long-term care, acute care, and community health. This role offers an opportunity to provide vital nursing services, support patient recovery, and improve quality of life for diverse patient populations. The LPN will be responsible for delivering safe, effective, and personalized care in accordance with healthcare standards and regulations. Experience with EMR/EHR system Point Click Care is preferred, along with proficiency in medical documentation and vital signs assessment.
Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and health history documentation.
- Administer medications accurately following physician orders while adhering to HIPAA and infection control protocols.
- Support patients with complex needs including dementia care, Alzheimer's care, memory care, hospice care.
- Ensure accurate medical documentation using EMR/EHR system PCC; maintain detailed records of intake, medication administration, and patient progress.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN).
- Proven experience in diverse clinical settings such as long-term care facilities, hospitals (including trauma centers), emergency rooms (ER), urgent care clinics, or home health environments.
- Knowledge of physiology, anatomy, medical terminology, pharmacology related to pain management and acute pain protocols.
- Strong skills in patient assessment, vital signs measurement, medication administration.
- Experience working with special populations such as geriatrics (dementia & memory care.
- Ability to work independently while collaborating effectively within a healthcare team; excellent communication skills are essential.
