Responsibilities
- Evaluate and treat residents under Medicare, Medicaid, and commercial payer guidelines
- Conduct comprehensive assessments for swallowing, speech, cognitive-communication, and language deficits
- Develop and implement individualized treatment plans with functional, achievable goals
- Document evaluations, daily therapy sessions, progress notes, and discharge summaries accurately and timely
- Revise treatment plans based on resident performance and progress
- Educate residents, families, and caregivers on therapeutic exercises, safe swallowing, and carryover strategies
- Collaborate with nursing staff, interdisciplinary team members, residents, and families to maximize functional outcomes
- Participate in care conferences, facility meetings, and quality improvement initiatives
- Maintain knowledge of community resources and assist with discharge planning when needed
- Engage in continuing education and provide in-service training as required
- Adhere to professional, ethical, regulatory, and facility standards
- Maintain expected productivity levels and demonstrate strong clinical judgment, communication, and teamwork
Qualifications
- Master's Degree in Speech-Language Pathology
- Certificate of Clinical Competence (CCC-SLP)
- Active Tennessee SLP license
- Current BLS / CPR certification
- Strong observational, verbal, and written communication skills
