Job Summary:

The Case Manager provides participants with a comprehensive array of education, life skills, occupational skills training, work experience and internships, mentoring, case management, scholarships, and career development services to promote and support educational and work attainment. Experience in both GED preparation and tutoring 8-12 students, helping students for the GED examination and enhancing academic skills after school.

General Duties:

  • Responsible for documenting case notes, maintaining accurate and up-to-date files, and identifying and addressing barriers to success. The Case Manager will make appropriate linkages and referrals for community services, facilitate goal setting, implement follow-up, and follow through with coordinated services plans. The Case Manager will also advocate on behalf of participants in community settings.
  • Network and build positive relationships with students, parents, and school consultants.
  • The Case Manager will facilitate the intake and assessment process. Network and build positive relationships with area educational/training systems. Facilitate work readiness workshops for employment and academic development, college readiness, and success.

Specific Duties:

  • Participates in the interview screening process to assess the potential participants for the program. Develop the questions for interviews; advise and explain regulations, rules, policies, and procedures to participants. May determine participant eligibility for services; apprises participant's obligations under program participation; completes applications and other forms accurately and thoroughly.
  • Analyzes information from interviews, tests, and other sources to develop short- and long-term participant goals; generates and implements individual employment plans.
  • Assesses a participant's education, work experience, skills, abilities, qualifications, and job interest.
  • Assesses participant readiness for job referral, classroom training, on-the-job training, and support services; attempts to match participants with available employment, training, or other opportunities/services.
  • Identify problems/barriers that hinder program participation, retention, and employability; assist participants in resolving and mitigating barriers to these areas by identifying the need for other services and benefits. Counsel participants regarding recommended solutions and suggested supportive services.

Required:

  • Bachelor's degree from an accredited college or university, with two years' experience in a Human Services setting. Or an equivalent combination of education and experience that provides the required knowledge, skills, and abilities.
  • Have nonprofit experience working with diverse populations.
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint) and Adobe Reader.
  • Have documentation, record-keeping, and outreach skills.
  • Excellent communication skills: interpersonal, verbal, and written.
  • Ability to conduct assignments independently.
  • Valid Driver's License, Insurance, and daily access to a vehicle.
  • Work flexible hours with occasional evenings and weekends.

Preferred:

  • Master's degree from an accredited college or university, with one year's experience in a Human Services setting.
  • Have nonprofit experience working with diverse populations.
  • Have documentation, record-keeping, and outreach skills.
  • Excellent communication skills: interpersonal, verbal, and written.
  • Prior experience facilitating group workshops.
  • Ability to conduct assignments independently.
  • Valid Driver's License, Insurance, and daily access to a vehicle.
  • Have a working knowledge of Microsoft One Drives' file management systems.
  • Working knowledge of Google Suite.
  • Have the ability to work flexible hours with occasional evenings and weekends.
