Description

Lifelong's Aging and Disability Services (ADS) Case Management Program (CMP) provides support and Medicaid case management services to enable clients to reside in the residence of their choice and avoid living in a skilled nursing facility. The ADS CMP Case Aide is responsible for program administrative and operational support, which includes monthly assessment paperwork and care plan support, ad hoc client and caregiver communication, resource finding - including medical equipment, and general paperwork and care plan-related mailing support. The position reports to the ADS Manager and requires the ability to communicate clearly both orally and in writing, maintain professional boundaries, and carry out effective time management with varying day-to-day activities and related workflow. This is a regular, full-time, non-exempt, union position.

Qualifications

Essential Duties:

  • Client Services:
    • Provides individualized care plan support to program beneficiaries as an extension of case management staff to stabilize clients' in-home care and ancillary services.
    • Helps clients connect to benefits, in coordination with case management staff, based on functional and financial eligibility to ensure quality continuous care.
    • Coordinates with case management staff, along with a variety of health care and social service providers and community partners, to assure care plan needs are met and arranges appropriate transitional care services.
    • Educates, supports, and promotes risk reduction with clients and their communities.
    • Facilitates medical equipment and environmental modification obtainment procedures including medical provider communication, documentation gathering, authorization, and delivery confirmation.
    • Maintains strict confidentiality of client records.
  • Quality Assurance:
    • Assists in administering monthly assessments and provides administrative support to case management staff by assuring that necessary service plan finalization steps and authorizations are completed according to quality standards to assure continuity of client care.
    • Completes required documentation across various databases.
    • Performs routine maintenance of client electronic files, accurately filing all documents, service receipts, correspondence, and carrying out file archiving.
  • Communication:
    • Communicates with all program staff, interagency staff, and community partners and providers as needed to assure client care plan needs are met.
    • Communicates with program beneficiaries, informal supports, and formal care providers as needed to perform duties of the position.
  • Administrative Support:
    • Carries out monthly care plan paperwork requirements and care plan approvals on behalf of program beneficiaries.
    • Processes intakes, client assignments, faxes, terminations and transfers.
    • Conducts database projects as assigned.
    • Processes incoming and outgoing departmental mail.
    • Records and distributes weekly departmental meetings and trainings.
    • Receives cross-training on relevant duties of case management and CLC staff to be available to cover those duties when necessary.
    • Carries out other project work and duties as assigned.

Core Competencies:

  • Familiarity with individuals who experience health disparities, including complex chronic health conditions, behavioral health barriers, chemical dependency, poverty, and stigma-induced isolation.
  • Comfort with working with individuals along the entire continuum of diversity, including but not limited to varying abilities, race, class, gender identity, and sexual orientation.
  • Demonstrated ability to organize and prioritize multiple projects and meet deadlines in a time-sensitive environment.
  • Excellent detail orientation with a high level of accuracy. 
  • Demonstrated flexibility and adaptability.
  • Proven clear understanding of professional boundaries.
  • Demonstrated skills and working knowledge of Microsoft Office suite of applications, including Excel and Word.
  • Experience working in electronic case noting within a data base and electronic document collection and storage.
  • Proven ability to communicate clearly and professionally in both verbal and written form.
  • Well-developed professional phone skills.
  • Proven ability to work independently and in a collaborative team environment.
  • Ability to respond quickly to change.
  • Well-developed ability to build strong working relationships with internal and external providers and partner agencies.
  • Demonstrated ability to work effectively under stressful conditions, exercising crisis management and de-escalation skills.

Education and Experience:

  • Bachelor's degree in social sciences, public administration, or a related field preferred. One year of professional social service experience preferred.
  • Experience working with government entitlement programs (e.g. Medicaid or Medicare funded programs) preferred.
  • Experience working with elderly population preferred.

Minimum Qualifications:

  • Must meet the below education & experience combinations:
    • Must have one year of in-person customer service experience and a high school diploma or its equivalent. Experience may be paid or volunteer.

Work Environment:

  • The position is primarily located in an office setting in Seattle, with the work occurring via a hybrid model of time in the office, time out in the field with clients, and time working remotely from home.
  • This position requires regular work at a desk and on a computer, combined with frequent interaction with clients both on-site and in the community. The role may involve travel to client homes, partner agencies, or community locations, which can include exposure to a variety of environmental conditions. Employees must be able to sit, stand, walk, bend, and reach as needed, and occasionally lift up to 20 pounds to transport materials or supplies. Work may occur in office, residential, or community settings, and may occasionally involve outdoor conditions or environments with moderate noise.
  • All positions that involve in person work with clients are required to undergo a TB test within 30 days of starting employment and annually, or as required by program.
  • Any offer of employment is contingent upon passing a criminal background check. Some programs may require additional background checks or frequency.
