Job Overview

Our established and fast‑growing Primary Care practice is seeking a dedicated Nurse Practitioner (NP) to join our clinical team. The ideal candidate is compassionate, reliable, and skilled in managing both acute and chronic conditions. This role involves providing comprehensive patient care, collaborating with our physician, and supporting a positive, efficient office environment.

Key Duties

• Perform comprehensive patient assessments, physical exams, and follow‑up visits
• Diagnose and manage acute and chronic medical conditions
• Order and interpret diagnostic tests
• Develop and implement patient treatment plans
• Provide patient education on preventive care, medications, and lifestyle management
• Document all encounters accurately and efficiently in the EMR
• Collaborate with the physician and clinical staff to ensure continuity of care
• Participate in quality improvement initiatives and office protocols

Qualifications

• Active and unrestricted Nurse Practitioner license in New York
• Board Certification in Family Medicine or Adult-Gerontology (preferred)
• Minimum 1 year of primary care experience (preferred)
• Strong clinical judgment and ability to work independently
• Excellent communication, professionalism, and patient‑centered care skills
• EMR proficiency and strong documentation habits
• Ability to thrive in a fast‑paced outpatient environment

Pay: $ $65.00 per hour
