Responsibilities:

• Provide direct patient care, including medication administration, wound care, and vital sign monitoring
• Develop and implement individualized care plans to meet the unique needs of each patient
• Collaborate with interdisciplinary teams to ensure seamless coordination of care
• Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
• Maintain accurate and timely documentation of patient care and progress

Requirements:

• Current LPN licensure in the State of Pennsylvania
• 0 years of experience minimum (recent graduates encouraged to apply)
• Strong physical and mental stamina to perform duties in a fast-paced environment
