Salary Range: $55,000 annually (non-negotiable)

- Master's degree in social work, Psychology, or a related field.
- Minimum two years' experience providing trauma-focused case management to high-risk youth and families.
- Knowledge of trauma-informed care principles and case management best practices.
- Willingness to work flexible schedules as deemed necessary including weekends and evenings.
- Must be proficient in Microsoft Office applications.
- Excellent organizational skills, dependent and responsive with attention to details and ability to prioritize multiple tasks.
- Ability to work effectively in a team environment, excellent interpersonal skills with a positive attitude, and maximum discretion when handling sensitive and confidential information.
- Customer service is oriented and sensitive to clients' immediate needs.
- Extended knowledge of community and social services.
- Must have access to a licensed, insured personal vehicle at all times during work hours for reliable transportation. Mileage reimbursement will be provided for the use of personal vehicles as part of work-related travel.
Gang Alternative, Inc.'s Collective Action for Addressing Trauma Program involves programming in the tri-county area, namely: Palm Beach, Broward, and Miami-Dade Counties. It is designed to effectively address community violence and collective trauma, using a comprehensive, integrated, and multi-sectoral approach to substance use prevention, violence prevention, and trauma-focused community engagement.
Essential Duties and Responsibilities- Conduct intake assessments to determine client needs, strengths, and barriers.
- Develop individualized care plans with measurable goals and objectives.
- Provide crisis intervention and emotional support using a trauma-informed approach.
- Advocate for clients with social services, healthcare providers, and other agencies and connect them to essential resources like therapy, housing and medical care.
- Educate clients on coping mechanisms and self-care strategies.
- Maintain regular contact with clients to assess progress and adjust care plans as needed.
- Facilitate support groups or workshops on trauma recovery and resilience.
- Maintain accurate and timely case notes, client records, and reports.
- Ensure compliance with agency policies, confidentiality laws, and funding requirements.
- Track client progress and report outcomes to supervisors or funding agencies as required.
- Working cooperatively with other GA teams and programs to maintain smooth continuity of care when cases are shared across teams.
- Become familiar with and utilization of outside resources as needed to provide comprehensive services to clients.
- All other duties as assigned.
