- Conduct comprehensive patient assessments and physical exams
- Evaluate hospice eligibility and facilitate patient transitions
- Lead goal-setting discussions with patients, families & care teams
- Implement palliative care plans to manage pain and symptoms
- Collaborate with physicians and interdisciplinary teams
- Provide family education & support on disease progression and care options
- Master's degree in Nursing & completion of an approved Nurse Practitioner program
- Active NYS RN & Nurse Practitioner licensure (or eligibility)
- 1+ years of clinical Nurse Practitioner experience (Palliative Care, Hospice, or Geriatrics preferred)
- CPR-BLS & valid NYS Driver's License preferred
