We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care and will be skilled in various nursing practices. This role involves working closely with patients in diverse settings, including hospitals, nursing homes, and assisted living facilities. The LPN will play a critical role in patient assessment, medication administration, and overall care management.
*Responsibilities*
* Provide direct patient care under the supervision of registered nurses and physicians.
* Administer medications and treatments as prescribed, ensuring compliance with established protocols.
* Conduct patient assessments, including vital signs monitoring and health history documentation.
* Assist in developing and implementing individualized care plans for patients.
* Collaborate with healthcare team members to coordinate patient care effectively.
* Maintain accurate medical records and documentation in compliance with HIPAA regulations.
* Educate patients and their families about health management and disease prevention.
* Support patients with activities of daily living (ADLs) as needed.
* Participate in discharge planning and follow-up care coordination.
*Skills*
Candidates should possess a variety of skills to excel in this role, including but not limited to:
* Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
* Experience in critical care, long-term care, assisted living, or home care settings.
* Knowledge of medical terminology, ICD-9/ICD-10 coding, and medication administration protocols.
* Strong patient assessment skills with an understanding of anatomy and physiology.
* Ability to perform procedures such as IV insertion, phlebotomy, catheterization, and tube feeding.
* Familiarity with specialized areas like geriatrics, pediatrics, hospice care, and memory care.
* Excellent communication skills for effective interaction with patients and families.
