Quality Healthcare Group is a small but growing home health care agency proudly serving our local community with compassion, professionalism, and personalized care. We primarily work with Pennsylvania's Community Health Choices (CHC) MCOs, such as PA Health & Wellness and Keystone First, and are seeking experienced, dependable, and flexible LPNs to join our team on a 1099 contractor basis.
Position Summary:
We are seeking Licensed Practical Nurses (LPNs) to provide skilled nursing services to clients in their homes. This is a 1099 contractor or W2 position, ideal for nurses seeking flexible, independent work with meaningful patient impact. Candidates should be clinically competent, timely with documentation, and comfortable traveling between client homes within a designated service area.
Responsibilities:
- Deliver patient-centered nursing care in accordance with individualized care plans
- Administer medications and perform treatments as prescribed
- Conduct assessments and communicate clinical changes to supervising RN or physician
- Complete accurate and timely clinical documentation per payer and agency requirements
- Provide patient and caregiver education regarding disease management, medications, and wellness
- Maintain compliance with all applicable regulations and CHC program standards
- Coordinate scheduling and visits in conjunction with agency support staff
Requirements:
- Active LPN license in the state of Pennsylvania
- Minimum 1 year of clinical experience, preferably in home health, community care, or long-term care
- Must be credentialed or willing to become credentialed with Keystone First and PA Health & Wellness
- Reliable vehicle, valid driver's license, and proof of insurance
- Current CPR certification
- Strong organizational and time management skills
- Ability to work independently while maintaining clinical excellence
