DEFINITION

The LPN is responsible for providing confidential addictions counselling, education, medical assessment and support to individuals, families and the community, residential facility and for promoting education of Medication Assisted Treatment (MAT).

Function: Responsible for providing education of Medication Assisted Treatment (MAT) and administering injections of Vivitrol.

Responsibilities:

  • Performs initial and on-going assessments of clients' needs, motivations, and backgrounds; prepares substance abuse bio-psychosocial assessment summaries with a diagnostic rationale and diagnosis consistent with the DSM V.
  • Monitor the health of patients and conduct residential medical questionnaires as needed to support ARNP.
  • Reviews screening and lab work for Medication Assisted Treatment (MAT).
  • Provide education to clients on benefits and side effects of MAT.
  • Administer injection on Vivitrol.
  • Responsible for maintaining original, accurate and up-to-date case records in a timely and thorough manner on each client consistent with program policy and the 65D-30, F.A.C.
  • Develop individualized treatment plans upon admission and reviews every thirty days (30 days).
  • Monitoring, evaluating, and recording client progress with that of on-going treatment goals. Enter daily group notes and individual counseling notes weekly.
  • Examine issues with participant including but not limited to, substance use, trauma, domestic violence, anger management, careers, depression, relationships, LGBTQ issues, self-image, stress and suicide.
  • Prepare participant for program completion by means of a written transition plan.
  • Responsible for scheduling family counseling as needed and facilitating family counseling session.
  • Ensure program participants are receiving regularly scheduled visitation with their child(ren).
  • Conduct observed urinalysis.
  • Responsible for completing and turning in on time all required client documentation to the Program Director (this includes closed files, census rosters and referral source monthly reports) so that they may be reviewed and submitted by the 5th day of each month.
  • Establish and maintain good rapport and effective working relationships with families, visitors, partner providers and other employees.
  • May be subpoenaed to appear and testify in court.
  • Ensure compliance with delivering only Evidenced Based Practices and approved program group counseling curriculum.
  • Must be willing to accompany program participants to community events for volunteer hours and attendance of self-help groups.
  • Responsible for providing referrals to program participants, or families to community resources.
  • Ensure compliance with all appropriate professional standards, outcomes, licensing and certification requirements, and the funders' conditions for the delivery of program and clinical services
  • Responsible for conforming to all codes of ethics established regarding performance of duty, client interaction and confidentiality, etc.
  • Report to the Florida Abuse Registry any and all suspected cases of abuse, neglect or exploitation as required by D.O.P. #150-1.
  • Responsible for reporting to staff supervisor all pertinent information that would serve in the client's and/or agency's best interest. Required to complete incident reports within one (1) hour.
  • Must familiarize self with agency policies and procedures located on the agency network.
  • Must attend all weekly staff and supervision meetings and be able to present case staffing's.
  • Must be willing to work a flexible schedule including holidays and weekends between the hours of 8am and 8pm.
  • Maintain a clean and safe working environment. Responsible for assisting in maintaining the overall cleanliness, safety, security, repairs and upkeep of the facility in which the program operates.
  • Responsible for submitting purchase orders for supplies.
  • Required to be able to drive a 15 passenger van to provide transportation to clients.
  • May be required to drive your personal vehicle for meetings, appointments, conferences, errands, etc.
  • Performs other related tasks as assigned.

Required Knowledge, Skills and Abilities:

  • A thorough knowledge of substance use and mental health counseling techniques.
  • A thorough knowledge of group dynamics and the required skills to effectively facilitate group therapy.
  • Knowledge of substance abuse counseling and treatment principles, practices, and techniques.
  • Knowledge of current social, economic, and health problems of individuals with substance abuse problems.
  • Understanding of cultural awareness and sensitivity.
  • Must have thorough knowledge and understanding of human behavior.
  • An ability to work flexible hours including nights, weekends and holidays if necessary.
  • An ability to use initiative and creativity in effective problem solving.
  • Ability to work with clients of complex needs and from diverse populations.
  • Must be punctual and complete all assignments by assigned deadlines.
  • Good organizational skills.
  • Ability to work collaboratively and as part of a team.
  • Able to work collaboratively with multiple health and criminal justice professionals using tact, diplomacy, and discipline.
  • Excellent verbal and written communication skills.
  • Be willing to submit to random drug testing.

Environmental Conditions

The LPN may find their office to be busy with both scheduled and unscheduled clients. The LPN must be ready to respond quickly and effectively to many types of situations, including crisis situations. The LPN must adhere to confidentiality standards and able to quickly secure Protected Health Information if interrupted. The LPN will also be invited to other environments that are stressful. Response to crisis situations can be unpredictable, emotional and threatening. The LPN must also be prepared to manage their own stress.

Sensory Demands

The LPN will be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves. The LPN may experience a number of unpleasant sensory demands associated with the use of alcohol and drugs, and the lack of personal care.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is occasionally required to stand, walk, sit use hands to finger, handle, or feel objects, tools, or controls; reach with arms; drive; climb or balance and talk or hear. The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.

Mental Demands

The LPN must monitor their own time and stress levels to ensure that they are able to effectively assist clients. The LPN may find that they are continually interrupted and may have to quickly assess situations to respond appropriately. The workload may be unpredictable and difficult to manage at times. The LPN may be placed in a situation where his/her own life may be threatened as well as life-threatening situations for others such as instances of attempted suicide.

Minimum Education, Experience and/or Training:

  • High School Graduate.

Or

  • Associates Degree from an accredited institution.

and

  • Licensed Practical Nursing License in good standing with the State of Florida, Division of Medical Quality Assurance.

Or

Must pass a required level 2 background screening every five (5) years.

Must maintain a Valid Florida Driver's License and personal automobile insurance.

Complete twenty (20) hours of state mandated training requirements detailed in the Florida Administrative Code 65D-30 each fiscal year.

Pay: From $24.00 per hour
