Treatment plans are based on individual needs and include Group, Individual and family-oriented systems of care, trauma-informed therapy, and a robust LIFETIME Alumni and aftercare program.
Position Type: Full time. Monday-Friday. 9-5 pm. 40 hours a week.
Qualifications
- Bachelor's degree in social work, psychology, or health and human services
- One year of experience working in substance abuse. (Preferred)
- Knowledge of community resources, Medicaid services, and social determinants of health.

Responsibilities
- The ideal candidate will have experience in a residential treatment center working with clients in substance use, particularly within acute care environments.
- This role involves providing comprehensive case mamangement services to individuals struggling with substance abuse, utilizing evidence-based practices to support recovery and rehabilitation
- Participate in weekly multidisciplinary meetings to share case updates
- Collaborate with prescribers, therapists, and nurses to support holistic care
- Document all case management and peer activities within 24 hours
- Ensure all actions directly support treatment goals
- Discharge Planning & Aftercare
- Initiate discharge planning
- Develop aftercare plans with outpatient providers and support networks
- Schedule follow-up appointments prior to discharge and provide warm handoffs
- Assist with housing, transportation, and recovery resources to reduce treatment disruption
- Identify and escalate barriers to treatment adherence
- Reinforce safety plans during crises and connect patients to emergency resources
- Document crisis-related activities for quality assurance and compliance
- Facilitate Group Sessions
Pay: $45, $55,000.00 per year
