Speech Language Pathologist

Job Responsibilities:

• Evaluate residents referred for speech, language, cognitive-communication, voice, and swallowing disorders.
• Develop and implement individualized treatment plans based on each patient's needs, goals, and tolerance.
• Provide direct therapy to improve communication skills, cognitive function, and safe swallowing abilities.
• Set realistic, achievable goals and track progress throughout the plan of care.
• Accurately and promptly complete documentation, including evaluations, progress notes, re-certifications, and discharge summaries.
• Educate patients, families, and staff on therapy techniques, communication strategies, and safe swallowing practices.
• Collaborate with all interdisciplinary team members to ensure patient-centered, coordinated care.
• Maintain compliance with HIPAA, confidentiality standards, and Federal Residents' Rights.
• Perform additional duties as assigned by the Director of Rehabilitation.

Requirements:

• Graduation from an accredited Speech-Language Pathology program (Master's required).
• Valid state SLP license and registration (CCC-SLP preferred but CFYs welcome).
