Salary Range: $22.14-$30.00/hour. The hourly rate for this position will be determined based on years of qualified experience.
Responsible to: RTF Clinical DirectorJob Summary:
The Transition Coordinator is responsible for aiding and expediting a client's successful discharge to a lower level of care, most often back to their family. They are responsible for establishing a relationship with a client and family upon admittance and establishing goals and barriers in relation to discharge. The Transition Coordinator is responsible to identify resources in the home community and begin to develop linkages. As discharge gets closer, the coordinator is responsible for ensuring linkages are in place. The Transition Coordinator carries the case in "after-care" for 30-60 days to make sure linkages are effective and client is stable in the home community. Essential Job Duties:
• Develop discharge plans and identify barriers with family either prior to intake or upon intake.
• Develop with therapist safety care plan for home time.
• Visit family home within 30 days; develop relationship with the family to engage them in discharge planning.
• Accompany children on therapeutic home visits to provide skill building, behavior management techniques, crisis de-escalation, etc.
• Promote positive trauma informed relationships with family.
• Identify all possible linkage options in the anticipated home community.
• Work with client developing activities and skill development that can be utilized in the home community.
• Coordinate with therapist home time and monitor home time with ongoing communication with family.
• Re-evaluate goals and objectives with team one time per month.
• Monitor client benefits with RTF management team.
• Assess service needs related to use of flexible funds; coordinates the use of service dollars as needed. Document spending and related goals for each interaction utilizing funds.
• Provide advocacy and case management with educational programs, mental health providers, legal system and other service providers.Skills:

• Ability to establish relationships both with clients and adults (parents)Competencies Template: ProfessionalEducation & Experience Required: - Bachelor's degree in human services or related field with 3 years of experience OR; - Master's degree in social work or related field with 2 years of experienceRequired License/Registration/ Certification: - NYS Driver's LicenseTraining: – New Hire OrientationPhysical Requirements: The overall nature of the position is sedentary requiring little physical effort with occasional light physical exertion required. There is little, if any, exposure to environmental conditions.
• The constant physical demands of the position are standing, walking, talking, and hearing.
• The frequent physical demands of the position are sitting. climbing, repetitive motions, and eye/hand/foot coordination.
• The occasional physical demands of the position are lifting, carrying, pushing, pulling, balancing, stooping, crouching, reaching, handling, grasping, and feeling.The overall nature of the position is sedentary requiring little physical effort. However, the position requires that the individual is trained in TCI and has the ability to display the physical skills necessary to be certified.
