POSITION SUMMARY
The Wound Care Registered Nurse (RN) utilizes the nursing process to improve outcomes, to assure appropriate resource utilization, and continuity of care for individuals with altered skin integrity and other wound care issues. The Wound Care RN coordinates the care and services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers. The Wound Care RN will leverage clinical skills to facilitate the provision of care which includes the assessment, planning, intervention, and evaluation of patient care; this care is based upon cultural, spiritual, and age specific needs of the patient.
PRIMARY ACCOUNTABILITIES
- Coordinates inter-disciplinary plan of care based on current evidenced based practice. Assesses patient/family and collaborates with physicians and interdisciplinary team to identify clinical needs, while managing referrals of home health care, home medical equipment, and other services as appropriate in a timely manner.
- Documents all nursing care, change in condition, response to therapy, treatments, procedures, and referrals on patient record to meet standards.
- Serves as clinical preceptor to undergraduate nursing students.
- Proposes alternative treatment options as appropriate to ensure a cost effective and efficient plan of care and quality patient outcomes.
- Utilizes and cares for equipment and supplies economically.
- Contributes to the development, implementation, review of standards of care, and policies and procedures related to patients with wound care needs.
- Participates in research to improve practice and outcomes (e.g., Wound healing rates, Product Evaluation). Takes and active role in coordination of prevalence and incident studies per needs of the facility.
- Provides service to patients and families with sensitivity and respect for their needs, expectations, age, cultural, and individual differences, including education as directed by the plan of care.
- Contributes to the patient experience as evidenced by positive patient experience scores and/or written compliments by patients/others.
- Responds promptly to patient care per physician orders, while responding and anticipating patient and family needs while in the facility.
Work Experience
MINIMUM QUALIFICATIONS
- Education: Associate's degree in Nursing (ASN) or Nursing Diploma.
- Work Experience: Three (3) years' clinical experience.
- Licensure: Registered Nurse (RN) licensure in the State of Florida or endorsement.
- Certification: American Heart Association Basic Life Support Healthcare Provider Completion Card prior to start date and maintained.

- Possesses a working knowledge of available community resources.
- Demonstrates ability to recommend appropriate nursing interventions and alternatives in the care planning process.
- Demonstrates dependability, reliability, and flexibility.
- Demonstrates appropriate delegation and supervision of patient care activities within the health care setting.
- Demonstrates effective communication with other members of the health care team and promotes effective team functioning.
- Demonstrates effective and safe clinical practice as evidenced by skills fair demonstrations, completion of yearly competency assessment and age specific checklist.
- Demonstrates ability to recommend appropriate nursing interventions and alternatives in the care planning process.
PREFERRED QUALIFICATIONS
- Work Experience: Prior experience in wound care.
PHYSICAL REQUIREMENTS
- Physically agile; may require walking, standing, running, bending, stooping, kneeling, climbing, or crouching frequently.
- May occasionally include lifting or moving objects up to 50 pounds, with or without assistance.
- May require moving in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders, or overhead.
- May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift.
- May be exposed to inside and outside environments with varied temperatures, air quality, lighting, and/or low to loud noise.
- May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines.
- Communicating with others to exchange information.
- Visual acuity and hand-eye coordination to perform tasks.
- Workspace may vary from open to confined.
- May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
