Position Title: Physical Therapist
Department: Home Care
Supervisor: Home Care Director
Hours per week: Flexible (full or part-time hours)
Pay Range: The expected base pay for the position is $43–$58 per hour and may be increased based on other factors, such as language, certifications, etc.
Primary Function:
Under supervision and through a planned staff education and development program and in accordance with agency policies, the physical therapist carries out three major functions:
• Practices physical therapy in an out-of-hospital setting with regard for other health and social problems of patients and their families.
• Instructs and consults regarding rehabilitation programs. This includes participation in staff education.
• Utilizes and cooperates with appropriate community agencies.
Duties & Responsibilities:
The home care physical therapist performs the following functions to varying degrees depending upon his/her professional qualifications and experience:
• Implements specific written medical prescriptions in physical therapy and continues these programs under the direction of an attending physician.
• Teaches and demonstrates programs in physical therapy to patients and their family members (who are responsible for the patient's care).
• Communicates with attending physicians or clinics regarding the progress of patients.
• Recognizes patient and family health and social problems and cooperates with the nursing staff in providing total Agency services to patients and their families.
• Plans and works cooperatively with community personnel to help meet the needs of the patients and their families.
• Confers with supervisor, consultants, and administration as need arises on individual and family health and social problems.
• Attends monthly interdisciplinary staff meetings.
• Writes records and reports and uses them as evidence of care given, for continuity of patient care, in planning and evaluating services and for statistical purposes.
Minimum Basic Knowledge:
• Graduate of a physical therapy curriculum/program.
• Valid Massachusetts physical therapy license in good standing.
• Valid CPR certification.
Experience & Qualifications:
• Driver's license and reliable means of transportation to conduct home visits.
• Interest in working with the geriatric and/or a medically frail population.
• Willingness and interest in traveling to patients' homes.
• Prior therapy or rehabilitation experience a plus.
• Open to serving medical needs of a multicultural and diverse patient population.
• Bilingual language skills helpful.
