RN Case Manager

Alinea Family Hospice Care is a local family owned hospice care and we are now hiring a Full time RN Case Manager at our growing office for the Red Oak and surrounding areas.

Job Description:

Responsible for the provision and case management of nursing services to terminally ill patients in their home and facility settings as well as supportive services to family members in accordance with all applicable hospice policies and procedures.

Duties will include but are not limited to:

• Making skilled nursing visits to address and manage symptoms after hours
• Perform routine skilled nursing visits
• Perform evaluation as needed to determine eligibility
• Meet with families to provide education on hospice services
• Perform initial assessments for admission
• Pronouncement at time of death, postmortem visits
• Observe for patient changes and reports to physician and team
• Provide emotional support to patients, families and facility staff
• Provide education to patient', families and facility staff regarding end of life care
• Perform nursing procedures as needed as directed by physician Orders
• Provide dietary counseling
• Helps cover on call in a rotation with other nurses for assigned area.

Requirements/Qualifications:

• RN License a MUST
• Two (2) years in hospice, home care preferred.
• Basic Life Support Certification
• Must carry an In-state current "eligible" Driver License
• Valid Auto Insurance Required
• Must be able to able to understand, read, write and speak English

Job Type: Full-time

Pay: $82, $90,000.00 per year

Experience:
