The Nurse Practitioner will collaborate with physicians and other healthcare professionals to ensure comprehensive patient management.

Duties

• Conduct thorough patient assessments and physical examinations.
• Develop and implement individualized treatment plans based on patient needs.
• Manage medical records and documentation accurately to ensure compliance with healthcare regulations.
• Effectively manage a member panel up to the goal set by the organization.
• Participate in utilization review processes to optimize patient care.
• Collaborate with interdisciplinary teams for effective discharge planning.
• Provide education and support to patients and their families regarding health management.
• Stay current with best practices in family medicine and participate in ongoing professional development.
• Timely coordination of care among internal and external ancillary providers and hospitals to provide high-quality care between departments and facilities
• Provide patient education and counseling

Experience

• A valid Nurse Practitioner license is required.
• Experience in a clinical setting, particularly in family medicine, is preferred.
• Strong communication skills and the ability to work collaboratively within a healthcare team are necessary.

Pay: $102, $123,568.51 per year
