PRIMARY FUNCTION: the Vice President of VBC Program Management is responsible for driving the execution, measurement, and optimization of all value-based care (VBC) initiatives across the enterprise. This role serves as the operational and analytical bridge between Clinical Leadership, Payer Relations, andTechnology, ensuring that VBC programs are tracked, measured, and continuously improved to deliver measurable results in quality, engagement, and financial performance.

The VP, VBC Program Management will identify opportunities to improve quality and financial outcomes, translate business needs into technical requirements, and ensure successful execution of enterprise-wide VBC initiatives.

SUPERVISORY RESPONSIBILITIES: This person is responsible for non-clinical management staff and regularly presents to senior stakeholders internally and represents PAFC externally at the highest levels.

ESSENTIAL FUNCTIONS OF THE JOB:

  • Program Oversight & Tracking

  • Lead enterprise tracking and governance of all value-based care initiatives, ensuring visibility into timelines, milestones, outcomes, and return on investment.

  • Track VBC program dashboard summarizing initiative status, key metrics, and performance against targets.
  • Partner with VBC Clinical Performance and Payer Relations teams to evaluate the impact and effectiveness of each program.

  • Operational & Performance Management

  • Oversee risk capture, SDOH incentive programs, and VBC workflows to ensure consistent execution across markets.

  • Identify and prioritize operational improvements to drive efficiency, compliance, and alignment with payer and clinical goals.
  • Monitor engagement and outcome metrics for all enterprise programs, ensuring initiatives translate into measurable improvements in quality and cost.

  • Analytics, Insights & Reporting

  • Collaborate with analytics and data science teams to define technical and analytical requirements for accurate tracking of VBC initiatives.

  • Translate business and clinical goals into measurable data models, dashboards, and reports.
  • Evaluate performance trends and provide actionable insights to leadership on gaps, risks, and improvement opportunities.

  • Cross-Functional Leadership & Integration

  • Serve as the program management hub connecting clinical, payer, operational, and technology functions.

  • Partner with the VP, Clinical Performance to translate strategies into operational workflows and measurable outcomes.
  • Collaborate with the VP, Payer Relations to ensure payer expectations and incentive structures are operationalized and tracked.
  • Ensure all programs have clear owners, success metrics, and reporting cadences.

  • Leadership & Development

  • Build and lead a high-performing team focused on operational excellence, analytics translation, and value realization.

  • Foster a culture of collaboration, accountability, and innovation within the VBC operations function.

EDUCATION & LICENSURE

Bachelor's degree in engineering, Business, Healthcare Administration, or a related field

Experience:

  • 10+ years of progressive experience in value-based care operations, population health, or payer/provider partnerships.
  • Demonstrated success in managing risk adjustment, MLR improvement, and VBC incentive programs.
  • Proven ability to bridge clinical, operational, and technical teams to drive measurable outcomes.
  • Experience defining system requirements and collaborating with product or technology teams to implement workflow automation and data solutions.
  • Strong analytical, financial modeling, and program management skills.
  • Exceptional communication and stakeholder management abilities.
