We’re looking for a compassionate and motivated *Physical Therapist* to join our Home Health team in *Livingston County*. You’ll visit patients in their homes, provide one-on-one care, and help them regain strength, mobility, and independence. If you enjoy working independently while still being part of a supportive team, this role is a great fit.
*Responsibilities*
* Evaluate patients in their homes and identify their physical needs.
* Create personalized treatment plans based on patient goals.
* Provide therapy such as mobility training, strengthening exercises, and pain-relief techniques.
* Teach patients and families safe exercises and ways to continue progress at home.
* Document all visits and progress in the EMR system.
* Communicate with nurses, case managers, and other team members to ensure quality care.
* Follow doctor orders and update the physician on patient progress when needed.
* Support safe discharge planning and continuity of care.

License/Certification:
* Physical Therapy License (Required)
* Driver's License (Required)
